Spreading and Synchronous Depressions of Cortical Activity in Acutely Injured Human Brain

Abstract: Background and Purpose-Cortical spreading depression (CSD) has been much studied experimentally but never demonstrated unequivocally in human neocortex by direct electrophysiological recording. A similar phenomenon, peri-infarct depolarization, occurs in experimental models of stroke and causes the infarct to enlarge. Our current understanding of the mechanisms of deterioration in the days after major traumatic or ischemic brain injury in humans has not yielded any effective, novel drug treatment. This study s… Show more

“…An entirely different scenario may apply in the human brain acutely injured for example by ischemic stroke, brain trauma, or subarachnoid or intracerebral hemorrhage (ICH) (Hansen and Lauritzen, 1984;Strong et al, 2002;Fabricius et al, 2006;Dreier et al, 2006;Dohmen et al, 2008). This has been studied recently in patients who underwent neurosurgery including craniotomy because of their acute brain condition, by using cortical electrode strips similar to the technology applied in epilepsy surgery.…”

“…It is accompanied by reversible suppression of neuronal activity, failure of ion homeostasis and changes in the cerebral blood flow (CBF) without neuronal degeneration. Spreading depression was reported to be generated in several neurological disorders, including migraine (James et al, 2001), brain trauma (Strong et al, 2002;Fabricius et al, 2006), and ischemic stroke (Strong et al, 2002;Fabricius et al, 2006). The initiation and propagation of SD are known to trigger disruption of membrane ionic gradients, massive potassium efflux, and glutamate release in neurons as well as glial cells.…”

“…Moreover, recent clinical studies show that CSD or CSD-like events appear around the site of brain injuries in human patients suffering from acute brain trauma (Strong et al, 2002), intracerebral or subarachnoid hemorrhage (Dreier et al, 2006;Fabricius et al, 2006), and malignant hemispheric stroke (Dohmen et al, 2008).…”
