Spread of carbapenem-resistant Acinetobacter baumannii carrying a plasmid with two genes encoding OXA-72 carbapenemase in Lithuanian hospitals

Abstract: The spread of carbapenem-resistant A. baumannii in Lithuanian hospitals is strongly associated with strains belonging to ECII and carrying a GR2 plasmid encoding two blaOXA-72 genes. The genetic environment of pAB120 supports the role of site-specific recombination associated with the acquisition of carbapenem-hydrolysing class D β-lactamases.

“…OXA-72 is a single-amino-acid variant of OXA-24/40 (G224D). In contrast to OXA-23, which is the most common acquired carbapenemase in A. baumannii worldwide, OXA-72-producing isolates have been reported in only a few countries, including Brazil, Colombia, Taiwan, France, Croatia, Poland, Italy, Lithuania, and Sweden (19,21,(23)(24)(25)(26)(27)(28)(29)(30). The clonal spread of OXA-72-producing A. baumannii isolates was previously reported in the Balkan region, in a Croatian university hospital (24).…”

“…We have performed a PCR-based search for TA systems, validated in this work, with a collection of clinical A. baumannii isolates (n ϭ 476) from Lithuanian hospitals. Most of these isolates belonged to the multidrug-resistant clonal lineages known as European clone I (ECI) and ECII; others were sporadic or nontypeable isolates (N/S) according to a previous pulsed-field gel electrophoresis (PFGE) analysis (24).…”

“…Furthermore, according to our recent observations, these TA modules are harbored by the newly observed ϳ11-kb plasmid pAB120. This small plasmid is prevalent in carbapenem-resistant clinical A. baumannii ECII isolates and carries two copies of the gene for OXA-72 ␤-lactamase, conferring resistance to carbapenems (24). These observations suggest a probable role of plasmid-borne A. baumannii TAs as plasmid stabilization systems contributing to the evolution of antibiotic resistance in this opportunistic pathogen.…”
