Spontaneous third ventriculocisternostomy in an infant with obstructive hydrocephalus

Abstract: ✓Spontaneous ventriculocisternostomy, the spontaneous communication between the ventricular system and the subarachnoid space, is rare. The authors report a case of an infant with obstructive hydrocephalus who developed a spontaneous third ventriculocisternostomy. The infant was initially evaluated for progressive ventriculomegaly and increasing head circumference (HC). During follow-up, the patient's HC began to follow percentile lines and magnetic resonance (MR) imaging demonstrated a reduction of th… Show more

“…Temporary CSF diversion is included in the protocols of many departments, with the objective of reducing the risk of recurrence, whether an endoscopic endonasal approach or an intracranial approach is chosen [4,18,28]. Measurement of CSF pressure after the repair and immediate ventriculoperitoneal shunting if necessary are additional to these protocols [4,7,28]. The lumboperitoneal shunt provides a solution when direct methods of treatment have failed, and this should be considered as a primary mode of treatment in elderly patients [1].…”

“…[18–10] Previously reported cases were noted to have occurred secondary to obstructive hydrocephalus caused by neoplastic disease or by benign stenosis of the aqueduct, mostly in adults. [25781011] There were a few reports of spontaneous ventriculostomy in infants due to aqueductal stenosis involving the third ventricle[36] and only one occurring as a consequence of congenital stenosis of the foramen of Monro. [1] Spontaneous ventriculostomy may occur in a number of locations, but only four reports refer to the atrium of the lateral ventricle.…”
