Spontaneous spinal epidural hematoma as the initial presentation of leukemia

Abstract: We present a case of a 55-year-old male with progressive neurological deficits that appeared dramatically. MRI detected a spinal epidural hematoma at the cervicothoracic junction and blood tests showed leukocytosis, mild anemia, and thrombocytosis. Spontaneous spinal epidural hematoma (SSEH) as the initial presentation of leukemia was diagnosed. Urgent posterior decompression was performed after 28 h from acute onset of backache, and the patient experienced remarkable improvement in neurological findings.

“…23 It is usually isointense on T1-weighted and hyperintense on T2-weighted MRI in the first 24 hours following symptom onset, and hyperintense on both T1-weighted and T2-weighted MRI after 24 hours. 1,13,[24][25][26] Enhanced MRI is not essential but may help exclude other epidural lesions. Spinal CT scans, especially three-dimensional CT scans, can also be useful where MRI is contradicted or unavailable.…”

“…Chronic myeloid leukemia has been reported rarely as a cause of spontaneous hematoma in various sites, i.e., chest wall [ 12 ], soft tissue [ 13 ], retroperitoneal spontaneous iliac psoas muscle [ 14 ], mediastinal hematoma and hemothorax [ 15 ], spontaneous epidural hematoma [ 16 ], and following bone marrow biopsy [ 19 ].…”

“…Three of the patients had a haematological malignancy. Two had chronic myelogenous leukaemia with elevated immature white cell and platelet counts [3,5] and the other had multiple myeloma in the spine with normal blood parameters but had received prophylactic low molecular weight heparin prior to onset of haematoma [2]. The authors suspected that the tumour may have caused epidural inflammation and epidural venous plexus fragility resulting in the haematoma.…”
