Spontaneous Solitaire™ AB Thrombectomy Stent Detachment During Stroke Treatment

Abstract: Spontaneous Solitaire™ stent retriever detachment is a rarely defined entity seen during stroke treatment, which can result in a disastrous clinical outcome if it cannot be solved within a critical stroke treatment time window. Two solutions to this problem are presented in the literature. The first is to leave the stent in place and apply angioplasty to the detached stent, while the second involves surgically removing the stent from the location at which it detached. Here, we present a case of inadvertent ste… Show more

“…A less commonly reported complication is breakage and retention of the thrombectomy device. This can not only lead to the failure of thrombectomy, but also leave a thrombogenic foreign object inside the artery, which can potentially lead to clot propagation [4] and stroke progression.…”

“…The manufacturer's instructions for use (IFU) recommend re-sheathing the proximal marker prior to Solitaire FR device retrieval, presumably to lower the risk of separation, although a case of detachment despite these instructions has been reported [22]. …”

“…Potential causes are vascular pathologies such as arterial stenosis, tortuosity and wall calcification; increased number of stent passes during SAT; and stent structural features. 6,7 Two stent detachments occurred in our study during the use of the Solitaire FR stent, which is a detachable stent. In the first case, stent detachment occurred at the dissected basilar artery with accompanying focal stenosis and, in the second case, at the MCA, which was assumed to be related to stent structural features (Figures 2 and 3).…”

“…Two solutions are suggested in the literature to overcome this complication: leaving the stent in place and performing angioplasty, or removal of the detached stent and replacement with another thrombectomy stent within the treatment time window (Figure 4). 6,7 Arterial dissection Iatrogenic dissection is a type of traumatic dissection that may occur during SAT. However, there are no known discussions in the literature of the relationship between the catheter or stent type and dissection.…”
