Objective To assess the impact of repetitive movements of the thumb caused by playing a smartphone game continuously for 30 min. Methods This study recruited healthy volunteers. They were instructed to play a game called Subway Surfers, which ran continuously. Subjects played the game for 30 min. The function of the abductor pollicis brevis and extensor pollicis brevis muscles was assessed by surface electromyography and the signals were obtained at 10 min, 20 min and 30 min. The median frequency (MDF) was used as an indicator of muscle fatigue. A visual analogue scale (VAS) was used to assess the subjective discomfort of the volunteers. Results Twelve subjects participated in this study. The MDF of the abductor pollicis brevis and extensor pollicis brevis muscles decreased significantly over the test period. The MDF of the extensor pollicis brevis had decreased significantly by 10 min after the start of the gaming session, while the abductor pollicis brevis had decreased significantly by 20 min. The VAS scores significantly increased after 30 min of continuous gaming. Conclusions This study suggests that playing continuous games on a smartphone might result in chronic muscle injury. Continuous gaming time should be kept below 20 minutes.

IntroducciónLa rotura espontanea del tendón extensor largo del pulgar (ELP), es una patología infrecuente dentro de la patología de la mano. En la literatura han sido reportados distintos factores asociados dentro de los cuales los más frecuentes son la fractura de radio distal con una incidencia de hasta un 5% de las fracturas, la artritis reumatoide y la inyección sistémica o local de corticoides.1-4 Las actividades repetitivas con la extremidad, como son algunos tipos de danza y oficios, también han sido consideradas factores de riesgo para el desarrollo de esta patología. 5-7 Otro factor asociado es la tendinosis en la zona del tubérculo de Lister debido a un efecto compresivo, donde incluso se ha planteado la descompresión profiláctica. 8Palabras Clave ResumenLa rotura espontanea del tendón extensor largo del pulgar (ELP), es una patología infrecuente existiendo casos reportados en la literatura donde no se logra encontrar factores predisponentes. El manejo quirúrgico suele realizarse utilizando una técnica de transposición tendínea del tendón del extensor propio del índice. En la actualidad, la técnica anestésica de WALANT "Wide Awake Local Anesthesia with No-Torniquet" ha sido de amplio desarrollo en la cirugía de la mano, sobre todo para la resolución quirúrgica de patología de tendones, con buenos y excelentes resultados. Se presenta un caso de un paciente con rotura espontánea del tendón ELP, que fue manejado con una transferencia tendínea utilizando la técnica anestésica WALANT. The surgical management is done by the proper index extensor tendon transposition. Nowadays, the WALANT anesthetic technique ("Wide Awake Local Anesthesia with NoTorniquet") has had a broad development in hand surgery, especially in surgery for tendon injuries, with good and excellent results. A clinical case is shown with a patient who had a spontaneous EPL rupture which was managed with a tendon transfer under the WALANT anesthetics technique.

Zusammenfassung Der Streckapparat ist ein kompliziertes Geflecht von 2 unterschiedlichen Systemen, n?mlich dem intrinsischen und dem extrinsischen, und wird in 9 Zonen eingeteilt. Verletzungen in der Zone 1?3 k?nnen i.?d.?R. konservativ behandelt werden. Verletzungen in den ?brigen Zonen bed?rfen einer operativen Therapie. Die Dauer der Ruhigstellung richtet sich nach der Lokalisation der Verletzung und nimmt von distal nach proximal ab. Aufgrund enger r?umlicher Lagebeziehungen der Fingergelenke zum Skelett sind Begleitverletzungen sorgf?ltig auszuschlie?en und ggf. zu behandeln. Posttraumatische Adh?sionen der Sehnen k?nnen leicht zu unbefriedigenden Ergebnissen f?hren.
