Spontaneous multivessel coronary artery dissection with anomalous coronary artery

Abstract: SUMMARYSpontaneous coronary artery dissection (SCAD) is one of the rare causes of acute coronary syndrome in young healthy individuals especially women without having any conventional risk factors for coronary artery disease. We describe a case of 34-year-old healthy man with diffuse multiple SCADs who presented with acute coronary syndrome and was managed conservatively with an uneventful course on long-term follow-up. “…The most common coronary artery to dissect is the left anterior descending artery [ 1 , 2 , 8 ] . The LAD is affected in 75%, RCA in 20%, LCx in 4% and the LMCA in less than 1% of cases [ 4 ] . This case represented a rare occurrence of SCAD with multivessel involvement in which the LAD was spared.…”

“…The diagnosis of SCAD revolves around identifying characteristics of dissection either on conventional or CT coronary angiography [1,8]. Intravascular ultrasound (IVUS) can also be used to clarify the type of SCAD; however, it was avoided in this case due to fear of propagating the dissection, and clear coronary angiogram findings.…”

“…Intravascular ultrasound (IVUS) can also be used to clarify the type of SCAD; however, it was avoided in this case due to fear of propagating the dissection, and clear coronary angiogram findings. While the diagnosis is straightforward, the treatment options remain ambiguous [8]. Because this presents as an ACS, initial treatment involves immediate therapy for myocardial infarction.…”

“…A thrombolytic therapy should be avoided as it can increase extent of disease 10. To date, there is no definite treatment guideline in SCAD.…”
