Spontaneous DNA Synthesis in Rheumatoid Arthritis: Evidence of Enhanced Circulating Non‐T‐Cell Proliferation

Abstract: 3H-thymidine incorporation in cultures of peripheral blood mononuclear cells from patients with seropositive rheumatoid arthritis (RA) and healthy controls was measured in vitro in the absence of added stimulants. A significantly higher level of spontaneous DNA synthesis was found in cultures of mononuclear cells from patients with clinically active RA than from patients with inactive disease and normal controls. This activity was more apparent in 24-h cultures than in 72-h cultures. There was no correlation b… Show more

“…A number of explanations may be suggested for this lack of correlation, which contrasts with previous findings by others of a strong correlation between lymphocyte activation and disease activity (6,7). Serum IL-2R may accumulate over time, and thus may be less sensitive to fluctuations in clinical disease activity.…”

“…Serum IL-2R levels were comparable with those in healthy controls (data not shown) and lower than those in the RA patient group (P = 0.002) (Figure 2B). In 6 OA patients, SF were obtained at the time of serum sampling. Soluble IL-2R levels in these samples were generally lower than those observed in RA SF.…”

“…Interestingly, malnutrition [13] and severe sepsis [15] have been shown to be associated with an impairment in the monocyte release of interleukin 1 in vitro, which is in line with the present indication of an inverse relationship between SpP and interleukin 1 activity in infected malnourished patients. The finding that SpP is higher after 24 h than after 72 h of cell culture has previously been reported in patients with active rheumatoid arthritis [1], and is suggestive of a lymphocyte population with a high rate of DNA synthesis and a short life span. Consequently, characteristics of spontaneous as well as mitogen-induced lymphocyte proliferation may be overlooked using longer periods of cell culture.…”

“…Spontaneous DNA synthesis in lymphocytes has been reported to be elevated in patients with connective tissue disorders (27,28), allergy (29), infections (19,30), and malignant tumors (31). IL-6 in serum also exhibits elevation in acute and chronic inflammation and in septic shock and injury (32,33).…”
