Spontaneous Delayed Brain Herniation through a Subdural Membrane after Tumor Surgery

Abstract: Brain herniation through a subdural membrane is an extremely rare complication, but must be a differential diagnosis in patients with a known chronic subdural hematoma or hygroma and clinical deterioration, even in the absence of recent surgery. Urgent surgical intervention of the herniated brain is recommended to reduce the risk of permanent neurological damage.

“…A rare case of cerebral herniation through a surgical defect with coexisting SDH, post partial resection of high‐grade glioma, has been described. The majority of these type of cases is reported in the pediatric population, with little data in the adult population 8 . High‐grade gliomas are associated with raised intracranial pressure due to the tumor mass and vasogenic oedema 3 .…”

“…The majority of these type of cases is reported in the pediatric population, with little data in the adult population. 8 High‐grade gliomas are associated with raised intracranial pressure due to the tumor mass and vasogenic oedema. 3 Surgery is indicated to obtain tissue for histopathological diagnosis and achieve maximum safer resection; it has been reported that the incidence of complications increases where less than 50% of the tumor is resected as the residual tumor mass and associated edema can worsen in the postoperative period.…”

“…Radiotherapy with or without concurrent chemotherapy is one of the most successful and widely used treatments for NPC (Chan, 2010). Although it has contributed to increased survival rates, it frequently causes burdensome side effects, such as cognitive impairment , epilepsy (Rong, Yin, Wang, Zhang, & Peng, 2017), and even coma from a brain herniation (Van Dycke et al, 2013), which seriously affected the quality of life (QOL; Zhou et al, 2018).…”
