Introdução: A dissecção espontânea da artéria coronária (DEAC) é uma condição rara, mas potencialmente fatal, que requer compreensão aprofundada para o manejo clínico adequado. Objetivo: Este estudo apresenta uma revisão da literatura sobre DEAC, complementada por relatos de casos clínicos, visando analisar suas apresentações clínicas, métodos diagnósticos, opções de tratamento e desfechos associados. Materiais/sujeitos e métodos: Utilizando uma metodologia específica, incluindo busca sistemática na literatura e descrição detalhada dos casos clínicos, observou-se uma variedade de manifestações clínicas e abordagens terapêuticas. O levantamento bibliográfico foi realizado nas plataformas PubMed e Scielo. Resultados e discussões: Destacam a importância do diagnóstico precoce, as controvérsias na fisiopatologia e as lacunas no manejo clínico da DEAC. Considerações finais: Um entendimento aprofundado desta condição é crucial para otimizar o manejo clínico e melhorar os desfechos dos pacientes afetados.
