Background: The objective of this study was to explore the reasons why couples discontinue fertility treatment. Methods: A retrospective exploratory study was performed at the Leuven University Fertility Centre, a university hospital-based fertility center. Women who discontinued treatment between September 2000 and December 2001 were contacted in 2004–2005 by telephone for a standardized interview which covered 9 dropout reasons. For each reason, its importance on the decision to stop treatment was measured on an 11-point Likert scale. Results: On average, psychological burden (x = 5.96) had the highest impact on the decision to stop treatment followed by physical burden (x = 4.48) and female age (x = 3.64). Perceived lack of staff expertise (x = 1.84), negative impact on social contacts (x = 2.12) and financial burden (x = 2.16) had the lowest impact on the decision to stop treatment. Longer duration of infertility was significantly positively correlated with a higher rating of physical burden as a reason to discontinue treatment (r = 0.48; p < 0.05). Discussion: On average, psychological burden appears to be most frequently named as the number one reason to discontinue infertility treatment, whereas financial burden had the lowest impact. Longer duration of infertility is associated with more externalizing reasons to discontinue treatment.

A infertilidade interrompe um projeto de vida pessoal e do casal, produzindo sofrimento psíquico. O desenvolvimento das técnicas de reprodução assistida possibilitou a solução para casais que anteriormente não teriam alternativas de tratamento. Assim, a fertilização in vitro constitui uma nova fonte de esperança para ter um filho, mas, ao mesmo tempo, pode ser acompanhada de muitas dificuldades. O objetivo deste estudo foi comparar os estados emocionais de homens e mulheres submetidos à fertilização in vitro e verificar a relação entre estado emocional das mulheres e ocorrência de gravidez. Foram avaliados vinte casais submetidos ao primeiro ciclo de fertilização in vitro, entre o 5º e o 10º dia após a transferência dos pré-embriões. A avaliação foi realizada por meio de testes psicológicos e entrevistas. As mulheres apresentaram níveis mais altos de ansiedade e depressão que os homens. A autoestima delas mostrou-se mais baixa. Um quarto das mulheres que apresentaram sintomas psicoemocionais não engravidou, porém esta relação não foi significativa. Concluiu-se que as mulheres apresentaram mais ansiedade e sintomas depressivos que os homens e autoestima mais baixa. O grupo de mulheres com sintomas psicoemocionais apresentou tendência a não engravidar.

Indications to tubal reconstructive surgery in the era of IVF

In recent years, the treatment of tubal infertility has witnessed a shift from reconstructive surgery to in vitro fertilization. However, tubal surgery retains specific advantages, and appropriate preoperative evaluation allows improved selection of patients who are candidates for tubal reconstructive surgery by identifying the patients with good reproductive prognosis. Of pivotal importance in the selection of patients is the intratubal direct evaluation performed at salpingoscopy. Term pregnancy rates of approximately 70% and 65% may be obtained in patients with periadnexal adhesions and bilateral distal tubal occlusion, respectively, when a normal tubal mucosa is observed at salpingoscopy.
