Spontaneous cholecystoduodenal fistula - spectrum of complications

Abstract: Introduction Spontaneous cholecystoduodenal fistula is a rare complication of the gallbladder calculosis. Bowel obstruction is the complication in less than 1% of these patients. The pathognomonic triad (Rigler triad) of pneumobilia, small-bowel distention, and ectopic gallstones is typical for gallstone ileus. In only 1-3% of the patients with bowel obstruction by ectopic gallstone the localization of obstruction is in the duodenum, and it is called Bouveret syndrome. The rarest complication is a floating non… Show more

“…Untreated CDF can lead to gallstone ileus, perforation, necrosis, sepsis, or gallbladder carcinoma 5,8,9 . A complication unique to biliary fistulas is known as Bouveret's syndrome, where the stone migrates through the fistula and impacts in the bowel, which is diagnosed when Rigler's triad of pneumobilia, small bowel distension, and ectopic gallstone is present on imaging 9,10 . Traditionally, CDFs were managed with open surgery until the first laparoscopic approach in 1995 1 .…”

“…It has high success rate, with acceptable surgical morbidity and mortality [11]. It is still a matter of debate whether cholecystectomy and repair of the fistula should be performed, due to spontaneous closure of fistulas in some cases [9,12,13] Funding: This research received no external funding.…”
