Abstract: Although spontaneous CSF leaks have the highest recurrence rate of any etiology, prospective evaluation demonstrates high success rates with control of intracranial hypertension.

“…[12] Chaaban et al, on the basis of their 5-year prospective study on 46 patients with 56 spontaneous CSF leaks, concluded that successful treatment of elevated intracranial pressure in combination with endoscopic repair can provide high success rates (93% primary and 100% secondary) approaching that of other etiologies. [36] In our series of 11 patients with spontaneous CSF leaks, 4 patients had evidence of raised intracranial pressure and underwent a lumbar thecoperitoneal shunt placement. Of the two failures, one patient had an anteromedial sphenoid encephalocele, whereas the other developed CSF leak at a site different from the primary repair site following lumbar thecoperitoneal shunt malfunction.…”

“…[20]. A series by Chaaban MR et al, with a series of 46 patients over five years success following a single endonasal procedure was 93% and 100% with a second procedure [21]. Similar studies worldwide have shown a high rate of success following an endoscopic repair of more than 90% and are today considered the standard of care for CSF rhinorrhoea.…”

“…Rowe and Sarkies [6] evaluated 34 patients, 31 of which were female, and saw a positive relationship between obesity (defined as a BMI of 30) and IIH. Elevated CSF pressures have been documented postoperatively in patients who have undergone SCSF leak repair [2]. Bony sellar expansion can be found in some patients with IIH, presumably due to the elevated CSF pressures [4,5].…”

“…Idiopathic intracranial hypertension (IIH) is linked to spontaneous cerebrospinal fluid (SCSF) leaks, empty sella syndrome, and obesity with elevated body mass index (BMI)[1,2,3]. In addition to empty sella syndrome, some patients with IIH develop bony sellar expansion [4,5].…”
