Spondylodiscitis Caused by Viridans Streptococci: Three Cases and a Review of the Literature

Abstract: Three cases of spondylodiscitis caused by viridans streptococci were observed within the course of 1 month. Although streptococci have been reported as the third most frequent cause of spondylodiscitis after staphylococci and gram-negative bacteria, alpha-haemolytic streptococci are rarely seen. The three patients presented with symptoms of low back pain; they felt well and did not have a fever or chills. Laboratory examinations revealed inflammation. Further examinations such as scintigraphy, computed tomogra… Show more

“…are most frequently isolated. Weber et al [7] described that among a total of 1.368 patients with non-tuberculous spondylodiscitis (in 85.4% the infective agent was identified) the infective organism identified was a Staphylococcus in 48%, a Gram-negative in 38% and a Streptococcus in only 10%. In the other hand, Le Moal et al [4] observed in their study of 92 patients with endocarditis (15% with associated spondylodiscitis) a predominance of Streptococcus sp.…”

“…Prospective studies for the determination of the frequency of this association are still lacking. The most common clinical picture of this association is musculoskeletal symptoms preceding endocarditis diagnosis [4][5][6][7][8], but we report here another feature of this association: spondylodiscitis complicating endocarditis in its late course.…”

“…Spondylodiscitis is predominantly caused by Staphylococcus aureus [7]. In a study of 1,368 spondylodiscitis patients, the causative agent was identified in 85% of cases, with 48% attributed to Staphylococcus and only 10% to Streptococcus [9]. Gouliouris and co-workers reported a 5-20% prevalence of streptococci species (viridans type and β-hemolytic streptococci) in pyogenic spondylodiscitis [7].…”

“…9 Staphylococcus aureus is the most frequent cause of spondylodiscitis, followed by Streptococcus spp, Escherichia coli and Mycobacterium tuberculosis. 10 Until this report, there has been no previous report of spondylodiscitis and native valve endocarditis due to S. vestibularis in the literature.…”
