Background: Ankylosing spondylitis (AS) is a chronic, progressive, inflammatory disease of the spine and SI joints. Diagnostic criteria and treatments have continued to evolve, necessitating a historical compendium of AS and its management. This paper aims to review the historical context underlying the discovery of AS, as well as the major diagnostic and therapeutic discoveries in the last two centuries. Methods: A scoping review of the literature pertaining to AS was performed via the Pubmed, Scopus, and Web of Science databases. Future directions of AS treatments were assessed by querying the clinicaltrials.gov website. Results: The history of AS can be traced as far back as ancient Egypt (as evidenced by the discovery of its presence in ancient Egyptian mummies) to the late 20th century, when the inherited nature of AS was linked to a genetic factor, HLA-B27. Each discovery made throughout the years led to further investigations into the pathophysiology, diagnosis, and treatment of AS. The criteria to differentiate AS from rheumatoid arthritis were first reported in 1893. Since then, diagnostic criteria for AS have undergone a series of changes before the present-day diagnostic criteria for AS were ultimately determined in 2009 by the Assessment of Spondyloarthritis International Society. Conclusion: As the pathophysiology of AS is better understood, healthcare providers are able to diagnose and treat the condition more effectively. In particular, earlier diagnosis and multiple treatment options have facilitated efficient and more effective treatment.

Der mit entzündlich-rheumatischen Erkrankungen einhergehende Knochenverlust wird per definitionem den sekundären Osteoporosen zugerechnet [22,60]. Dabei resultiert die Osteoporose aus einer Störung des physiologischen "remodeling-Prozesses" durch erhöhte Knochenresorption oder einen verminderten Knochenanbau bzw. aus einer kombinierten Störung. Die sekundäre Osteoporose bei entzündlich-rheumatischen Erkrankungen stellt eine nicht selten anzutreffende und problematische Komplikation dar.Bei der rheumatoiden Arthritis gilt die gelenknahe Osteoporose als ein radiomorphologisches Frühsymptom und wichtiges diagnostisches Kriterium, wobei sie wahrscheinlich zytokinvermittelt hervorgerufen wird. Als Spätkomplikation wird eine generalisierte Osteoporose gesehen, die sich auch kortisolunabhängig manifestiert und wohl multifaktoriell bedingt ist [2,24].Bei der ankylosierenden Spondylitis (AS) wurde in den letzten Jahrzehnten vordergründig der Pathogenese der KnochenneubilZusammenfassung Im Vordergrund der Charakterisierung einer ankylosierenden Spondylitis (AS) stand in den letzten Jahrzehnten zunächst meist die Pathogenese der Knochenneubildung und der Verkalkung des Bandapparates. Trotz Ausbildung von Syndesmophyten stellt eine sekundäre Osteoporose eine häufige Komplikation dar mit spontanen als auch traumatischen Wirbelkörperfrakturen. In der vorliegenden Übersichtsarbeit werden mögliche und pathogenetisch relevante Faktoren der sekundären Osteoporose bei AS dargestellt. Mittels dualer Photonenabsorptionsmetrie (DEXA)-Verfahren sind Aussagen zur Knochendichte eher in gering ausgeprägten ankylosierten Stadien zu erwarten, während in Stadien mit fortgeschrittenen Ankylosen der quantitativen Computertomographie (QCT)-Methode -aufgrund selektiver Messung von Spongiosa und Kortikalis -eher Vorrang zu gewähren ist. Bis heute existieren keine einheitlichen und erfolgversprechenden medikamentösen Therapieoptionen zur Beeinflussung der sekundären Osteoporose bei AS. AbstractAnkylosing spondylitis (AS) is characterised by chronic inflammation and partial ossification. Nevertheless, vertebral compression fractures and traumatic fractures of the vertebrae due to osteoporosis are a common symptom in AS. An overview of various aspects of osteoporosis in AS is presented. Bone density measurements of the spine are best performed by quantitative computed tomography. Using the DEXA method, valid statements are more likely to be expected in less marked ankylosing stages of AS. Until now, no efficient therapy has been established.
