Spinal Stabilization in Duchenne Muscular Dystrophy: Principles of Treatment and Record of 31 Operative Treated Cases

Abstract: The aim of this study was to report results of prophylactic spinal stabilization in patients with Duchenne muscular dystrophy. There is still debate regarding the ideal instrumentation. A prospective study of a consecutive series of 31 patients stabilized with the ISOLA system from D2 to S1 will be presented. The mean follow-up was 22 months (range, 1-60 months). The evaluation of the Cobb angle and pelvic obliquity revealed the following: 1) Cobb angle: preoperation, 48.6 degrees (range, 22-82 degrees); posto… Show more

“…None of the patients complained of pain in the region of iliac fixation. A loss of correction was described by some authors with Luque or CD instrumentation [7,41], but not by others [15,22]. There was no loss of correction of scoliosis or pelvic tilt in our patients.…”

“…To our knowledge, all reports on the scoliosis correction in patients with DMD deal with instrumentation of the spine using hooks, wires or hybrid instrumentation with pedicle screws in the lumbar spine [6,12,15,20,22,28,38,43]. This is the first report about the use of pedicle-screw-alone fixation for this pathology.…”

“…However, most reports on scoliosis surgery in patients with DMD have dealt with spinal instrumentation using hooks, wires and hybrid constructs with lumbar pedicle screws [4,12,17,22,23,30], yet it is difficult to provide strong and stable fixation with hook and wire anchors in patients with DMD [23,38].…”
