Spinal Seeding of a Pilocytic Astrocytoma in an Adult, Initially Diagnosed 18 Years Previously

Abstract: Pilocytic astrocytoma (PA) is a slow-growing, well-circumscribed grade I glioma generally considered benign, with a low recurrence rate and an excellent prognosis following complete surgical resection. PA is the most common central nervous system glioma in the pediatric population and is rare in adults. We report a 26-year-old male with an intradural extramedullary PA at the thoracolumbar junction following subtotal cerebellar PA resection 18 years previously. Fifteen months after spinal PA resection, the pati… Show more

“…Several cases of PA dissemination have been reported after surgical intervention [17], [22]. This is less likely the case in our patient, since LD was already visible on the initial MRI examination, before any surgical manipulation of the tumor.…”

“…PMA is more aggressive than the classic PA and classified as WHO grade II [1]. Previous reports have shown a strong association between PMA and LD [22], [30], [31]. In addition, other reports indicate an association between PMA and intratumoral hemorrhage [7], [32], (25%) in PMA compared with PA (8%) [33], [34].…”

“…11 The metastatic potential of pilocytic astrocytomas 5,9,18 and oligoastrocytomas 16,33 has also been described. Metastasis by a low-grade, nonpilocytic astrocytoma has been rarely reported in the literature, although the first report was in 1930.…”

“…Pilocytic astrocytoma is a WHO grade I tumor with a good prognosis after complete excision. Although occasional leptomeningeal dissemination has been reported in the past, drop metastasis in a pilocytic astrocytoma is very rare [12]. The CNS neoplasms, which frequently show drop metastases, are primitive neuroectodermal tumors, medulloblastomas, ependymomas and high-grade gliomas.…”
