Spinal Osteoid Osteoma Revealed by Radiculopathy: Case Report and Literature Review

Abstract: Osteoid osteoma (OO) is a benign tumor that usually occurs in long bones of young males. We report a rare case of spinal OO in a 25-year-old woman, revealed by a sciatica. Spinal radiographs and computed tomography scan were normal, although performed at 6 months of symptom evolution. On magnetic resonance imaging, however, an important edema of the right transverse process of L5 vertebrae was depicted but was inconclusive. The diagnosis of OO was finally retained on a second computed tomography scan with thin… Show more

“…Though spinal osteoid osteoma occurs close to neurological structures, deficit rarely occurs due to its small size [14]. Radiculopathy has been reported due to reactive inflammation caused by tumour [15]. In our series, first and second case had no symptoms or signs of nerve irritation.…”

“…The common presentation of OO in a spinal location is a painful scoliosis or an isolated spinal night pain alleviated with the use of aspirin or nonsteroidal anti-inflammatory drugs [8] as in our case. Ankylosing Spondylitis is a chronic inflammatory rheumatic disease, the hallmark manifestation of which is inflammatory back pain, with or without peripheral joints involvement, and a close association with HLA B27.…”

“…Osteoid osteoma accounts for about 10% of benign bone tumors 1 . Most osteoid osteomas occur between the ages of 20 to 30 years, with a male to female ratio of 2:1‐3:1 2 . About 10% of osteoid osteoma occurs in the spine 3 , mostly involving the posterior elements of the spine 4 .…”
