Spinal Manifestation of Metastasizing Leiomyosarcoma

Abstract: In retrospect, the origin of the leiomyosarcoma that manifested in the thoracic spine of the patient must be reconsidered. Rather than a primary dural leiomyosarcoma, this tumor represented the first evidence of recurrence of a missed diagnosis of early-stage uterine leiomyosarcoma.

“…The differential diagnosis of leiomyosarcoma includes tumors such as fibrosarcoma, malignant fibrous histiocytoma and malignant peripheral nerve sheath tumor 6 . Refinements of the immunohistochemical techniques have enabled precise interpretation of the nature of the cells and because metastases mostly involve axial skeleton, ruling them out is an important diagnostic problem 7–9 . We describe here another case of primary leiomyosarcoma of the vertebra, and discuss the clinicopathologic features of this rare tumor.…”

“…It usually occurs de novo, but in some cases, a history of prior irradiation may be present (5). The tumor spreads most commonly by hematogenous route to the lung (6), thyroid (7), liver (6, 8), brain (9), and bone (6,10). Bronchogenic carcinoma, breast carcinoma, melanoma, and gastrointestinal and urinary tract tumors commonly metastasize to skeletal muscle, but it is an unexpected site of metastasis of leiomyosarcoma (5).…”

“…In the present case, multiple cervical and thoracic vertebrae were involved by osteolytic lesions. Given the rarity of primary spinal leiomyosarcoma (6,7) and the patient’s previous history, the lesions were considered to be of a metastatic type, which was then confirmed by the pathological examination.…”
