Spinal epidural hematoma due to anticoagulant therapy: a case report and literature review

Abstract: This case report is to enlighten the awareness on spinal epidural haematoma due to warfarin. A 58-year-old man who had been on prophylactic anticoagulant therapy after aortic valve replacement since 1998, had presented with acute onset of reduced sensation and weakness in both lower extremities 3 days prior to the admission to our centre. His prothrombin time (PT) was 46.8 seconds, international normalized ratio (INR) was 5.11, and activated partial thromboplastin time (APTT) was 167.6 seconds (control, 31.3-4… Show more

“…Some authors attribute the bleed to the venous epidural plexus that is located mainly at the dorsal aspect (3). This plexus is prone to the undulating thoracic and abdominal pressure thus any sudden increase may cause rupture (9). Other than that, the dorsal epidural space is filled with fatty tissue and is more readily displace compared to the anterior epidural space making it easier to spread to multiple levels (3).…”

“…As the majority of patients suffering from CSDH are elderly and considering the fact that they usually suffer from cardiovascular diseases, the surgeon always tries to keep the operation process as short as possible [1,11,12,15]. As the operation using 1 burr hole takes half the time required for a 2 burr hole operation, the present research seeks to make a comparison between these two methods.…”

“…However, reduction in compliance of lung and chest, increased compliance of the abdominal wall and stiffness of rib cage in addition to paradoxical movements of chest wall contribute to breathing difficulties (6)(7)(8). While, expiratory muscle altered function reduced cough effectiveness and mucus retention (9,10).…”
