BackgroundIntracytoplasmic Sperm Injection (ICSI) is an Assisted Reproduction Technique (ART) which offers the chance to conceive to patients presenting very low sperm counts (cryptozoospermia/severe oligozoospermia). Sperm freezing before the oocyte pick-up, can prevent from a lack of spermatozoa on the day of the ICSI. It can avoid the cancellation of the ICSI or the use of TESE (Testicular sperm extraction). The objective of this study was to analyse the practice of sperm freezing for these patients in our center over 8 years and the rate of use of these frozen sperms. We also compared the outcome of ICSIs with frozen versus ejaculated sperm.Material and methodsWe performed a retrospective epidemiological study between 2004 and 2011. We recruited all the patients having a sperm count below 1 Million/mL and who were waiting for their first ICSI attempt.Results169 patients were recruited: 84 cryopreserved their sperm before the ICSI (secured ICSI) while 85 did not (non-secured ICSI). Both groups were split in cryptozoospermia (<103 spermatozoa/ml): 19 and 17 patients respectively, very severe oligozoospermia (103–105/ml): 37 and 13 patients, and severe oligozoospermia (105–106/ml): 28 and 55 patients. The part of secured ICSI significantly increased from 29% during 2004–2007 to 74% during 2008–2011(p = 0.0029) and the frozen sperm was used in 5.9% of the cases. Median age was significantly higher in the non secured ICSI group (33.57 vs 35.52 for men, p = 0.0069 and 30.45 vs 32.26 for women, p = 0.025) but no significant difference was found in the outcome of the ICSI between frozen-thawed sperm and fresh ejaculated sperm.ConclusionSperm freezing before ICSI for severe oligozoospermic and cryptozoospermic patients significantly increased in our practice but the rate of use remain very low. F ertilite için gerekli olan spermin bazı durumlarda sınırlı olarak elde edildiği ya da gelecekte azalacağı bilindiği için spermin gerektiğinde kullanılmak üzere dondurularak saklanması gündeme gelmiştir. Fertilite ile ilişkili mücadele ve gayretler çiftlerin ruh sağlığını bozduğu için bu stresi azaltabilmesi açısından da sperm dondurma işlemi büyük önem arzetmektedir. [1,2] İlk olarak 1940'lı yıllarda veteriner hekimler tarafından uygulanan sperm dondurma işlemi spermlerin-80°C ile-196°C arasında gerektiğinde tekrar kullanılmak üzere saklanmasını içermekte olup insanlarda 1950'li yıllardan beri yapılmaktadır. [3] Başlangıçta kullanımı sadece intrauterin inseminasyon (IUI) için sınırlı olan sperm dondurma ile ilişkili endikasyonlar günümüzde ol-ABSTRACT Sperm cryopreservation involves the cooling of semen samples and their storage at-196°C in liquid nitrogen. This technique has been used widely since the 1960s to treat couples with infertility. Cancer is the major indication for sperm cryopreservation. However currently, the scope of clinical indications used for sperm cryopreservation has expanded widely. In this context, sperm cryopreservation is used in patients with retrograde ejaculation, severe olgoospermia, metabolic disorders, spinal cord injury, cranial tumors. Cryopreservation process involves cooling, freezing, and thawing steps. Cryopreservation of sperm may negatively affect on membrane lipid composition, acrosome status, sperm motility, vitality and DNA damage. To prevent of this negative effects several cryoprotectants have been used. In general the pregnancy rates after sperm cryopreservation are changes between 12% and 35.2%. In this review current advances have been evaluated in the era of sperm cryopreservation.

A rosszindulatú daganatos betegségek műtétes, sugár-, kemo-és biológiai terápiás kezelése káros hatással lehet a fiatal betegek későbbi fertilis képességeire. A javuló gyógyulási esélyek mellett egyre nagyobb hangsúlyt kap a betegek hosszú távú életminősége, így a túlélésen túl jövőbeli fertilitásuk megőrzése is célként fogalmazódik meg. Az onkológiai és fertilitásmegtartási törekvéseket az onkofertilitás kifejezés foglalja össze. Ez alatt nemcsak speciális nőgyó-gyászati műtéti eljárásokat értünk, hanem olyan beavatkozásokat is, amelyek az ivarsejtek, a nemi szervek vagy az embrió konzerválását célozzák. Az onkofertilitás mint kifejezés az Amerikai Egyesült Államokban született, de napjainkban már az egész világon interdiszciplináris tudományterületként egyre nagyobb teret hódítva válik egyre több szakember speciális érdeklődési területévé. A klasszikus onkológiai és nőgyógyászati szubspecialitások mellett az onkofertilitás ismeretanyaga is olyan szintre emelkedik, amelynek naprakész tudása az egyes modalitások szakembereitől már nem várható el. Az onkológus, a sugárterapeuta, a sebész és a meddőségi specialista eseti együttműködését az onkofertilitásra specializálódott szakemberek útmutatása teheti igazán hatékonnyá. Increasingly greater emphasis has been put on ensuring these patients' long-term quality of life, therefore, in addition to survival, preserving their prospective fertility is also among our aims. The term 'oncofertility' has been compounded from two expressions, 'oncology' and attempts to 'fertility preservation'. In gynecology, fertility preservation involves not only special surgical procedures but also interventions intended to preserve gametes, reproductive organs and embryos. Although the term 'oncofertility' was created in the United States of America, it has become an interdisciplinary field of science worldwide. By gaining grounds steadily, it has attracted special attention among experts. In addition to the subdivisions of classical oncology and gynecology, knowledge of oncofertility is broadening so rapidly that one cannot expect experts to be 100% up-to-date in one or another modality. It is special guidance by consultants in oncofertility that can make occasional co-operation among oncologists, radiotherapists, surgeons and infertility specialists really effective.
