Spectrum of Manifestations of Swyer-James-MacLeod Syndrome

Abstract: SJMS should be considered as a spectrum disease. Bronchiectasis is not a universal finding. The presence and type of bronchiectasis will influence clinical manifestations and prognosis. Patients without bronchiectasis or with cylindrical bronchiectasis had a lower incidence of pneumonia episodes than those with saccular bronchiectasis.

“…Although bronchiectasis is not present in all patients, it influences clinical course and prognosis. In their 13-case series, Lucaya et al reported 9 patients with bronchiectasis, 5 cystic and 4 cylindrical, and determined that pneumonia was more prevalent in cases with cystic bronchiectasis [20]. In the present case series, two cases had cystic bronchiectasis areas, whereas three cases had varicose and/or cylindrical bronchiectasis.…”

“…It usually follows a viral respiratory infection such as adenoviruses or mycoplasma pneumonia infection in infancy or childhood. [1][2][3][4] Swyer-James syndrome refers to an inability of the lungs to grow after a severe obstructive bronchiolitis. On CT scan, the affected lung shows small size and decreased vascularity, hyperexpansion and hyperlucency, and, rarely, bronchiectasis.…”

“…What could be seen in the past with pulmonary angiography or CT angiography can now be identified without iodinated constrast agents with v-HRCT and MIP reconstruction, that allow us to make a reliable diagnosis. [18][19][20][21] In fact with the advent of multidetector CT, we may obtain, with lower X-rays exposure, up to 64 slices (0.5 mm thin) in a single rotation and in a single breath hold, thereby offering a volumetric acquisition of data without artifacts. Additionally, upgraded computer hardware and software have improved image quality and offer a better two and three dimensional reconstruction.…”
