Spectrum of immune‐mediated inner ear disease and cochlear implant results

Abstract: Triaging IMIED cases based on presence and type of systemic autoimmune disease may aid in selecting a management strategy. Knowledge about the predictors of CI outcome will help clinicians select appropriate patients for CIs. In the setting of significant and irreversible hearing deficit, the restoration of hearing using a cochlear prosthesis may be appropriate earlier rather than later.

“…Pathophysiology of inner ear involvement in systemic autoimmune diseases is still unclear and may be related to circulating antibodies against a number of inner ear antigens leading to antibody-dependent cell-mediated cytotoxicity, the activation of the complement system, a direct action of cytotoxic T cells, or immune complex-mediated damage [5, 7, 8, 1014]. …”

“…There is growing interest for inner ear involvement in systemic autoimmune diseases [5, 6]; this condition should be considered in patients with audiovestibular dysfunction presenting a constellation of symptoms consistent with systemic autoimmunity or with a preexisting diagnosis of autoimmune disease [7, 8]. Inner ear involvement in systemic autoimmune diseases should be distinguished from primary autoimmune inner ear disease, a condition in which the immune response acts directly against the inner ear [6, 7].…”

“…Results shown in this study, based on PTA, indicate that CI implanted in an ear with P-SNHL is an effective, safe measure to provide stable audibility from the first month of activation. This piece of information is extremely important for the group of patients with AHL because an objective of bimodal adaptations is to obtain better sound perception and ensure hearing through one of the ears, in case the contralateral ear may suffer some type of degenerative pathology, as could potentially be the case with bilateral Meniere's disease [15] , autoimmune hearing loss [16] , otoesclerosis [17] , or some labyrinth malformations [18] . The stable PTA results had already been described by other authors [19,20] , and their homogeneity, regardless of the initial HLs, can be explained by several reasons: 1.)…”
