Spectral-Domain Optical Coherence Tomography Following Nd:YAG Laser Membranotomy in Valsalva Retinopathy

Abstract: SD-OCT reveals formation of a premacular cavity following laser puncture for premacular hemorrhage in Valsalva retinopathy, which may persist despite successful drainage of blood. This may later result in metamorphopsia in certain patients.

“…In selected cases, the presence of the detached posterior hyaloid face at the area of the haemorrhage can be picked up, suggesting that the blood is sub-ILM and not subhyaloid. 5,6 When two layers are not evident, as in our case, the presence of a dome-shaped convex cavity with a hyper-reflective anterior layer on OCT corresponding with the ILM suggests that the haemorrhage is sub-ILM. 6 A review of existing literature revealed that OCT evaluation of pre-macular haemorrhage has been studied following Valsalva retinopathy; however, there is no report on that following anaemic retinopathy.…”

“…Moreover, the proximity to the retinal surface presents an additional problem when using the laser. Complications of Nd:YAG laser membranotomy include creating a macular hole or persistent premacular cavity, retinal detachment and epiretinal membrane formation [13,14]. Nd:YAG laser can be useful in the treatment of non-dense and non-coagulated premacular haemorrhages, and when the location of the blood is subhyaloid, but not when there is blood under the ILM.…”
