SPECT study with I-123-Ioflupane (DaTSCAN) in patients with essential tremor. Is there any correlation with Parkinson’s disease?

Abstract: Our results do not support the hypothesis of a link between essential tremor and Parkinson's disease. However, it appears that ET patients have a small degree of striatal dopaminergic degeneration. If this is due to alterations in the nigrostriatl pathway or of other origin it is not clear. Follow-up studies of essential tremor patients are warranted to assess progression of disease and to understand better the possible cause for striatal dopaminergic degeneration.

“…Moreover, two studies reported a mild reduction in striatal dopamine transporter binding in 60 ‘pure’ essential tremor patients compared to 59 healthy controls although less severe compared to 80 Parkinson's disease patients (Gerasimou et al, 2012; Isaias et al, 2008). The reduced dopaminergic uptake pattern differed between the studies.…”

“…Most scintigraphic studies looking into the basal ganglia found no dopaminergic deficits in essential tremor (Antonini et al, 2001; Asenbaum et al, 1998; Benamer et al, 2000; Breit et al, 2006; Brooks et al, 1992; Doepp et al, 2008; Isaias and Antonini, 2010; Wang et al, 2005) while two studies measured slight abnormalities (Gerasimou et al, 2012; Isaias et al, 2008). One study reported iron accumulation in the basal ganglia (Novellino et al, 2013).…”

“…This finding can help differentiate parkinsonian syndromes (i.e. PD, multiple system atrophy, progressive supranuclear palsy) from essential tremor, or dementia with Lewy bodies (which overlaps diagnosis of PD) from Alzheimer’s disease (Bajaj et al, 2013; Gerasimou et al, 2012; Oliveira et al., 2015; Thiriez et al, 2015). However, it is important to realize that decreased 123 I -FP-CIT identifies the loss of dopaminergic neurons, which is not specific for PD.…”
