Specific IgE against the house dust mite allergens Der p 5, 20 and 21 influences the phenotype and severity of atopic diseases

Abstract: Background: House dust mites (HDM) are among the most important sources for airborne allergens with high relevance for atopic diseases. Routine tests contain only 4 of 32 registered allergens of Dermatophagoides pteronyssinus. Clinical relevance and pathomechanistic properties of many allergens are not well understood. Objective: The association of several HDM allergens with allergic rhinitis, allergic asthma, and atopic dermatitis was investigated to identify allergens with biomarker potential and to transfer … Show more

“…However, testing solely for these components would miss sensitization in 2.70% (only sensitized to Der p 5, Der p 7, Der p 21) and overlook 37.21% sensitized to them in addition to major allergens. Recognizing mid‐tier/minor components like Der p 5, Der p 7, Der p 21 is crucial for accurate diagnosis, considering their unique immunological properties 20–22 . As previously described by Posa et al 8 .…”

“…22,23 However, our study did not yield distinct sensitization profiles associated with specific diseases, underscoring the potential influence of geographic and demographic factors on sensitization patterns. 4,24 One particularly intriguing observation was the progressive increase in the number of sensitized mite allergen molecules, concurrently with increased sensitization to other aeroallergens as age advanced. Notably, the substantial degree of selective tropomyosin and arginine kinase sensitization suggests that the primary source of sensitization likely extends beyond mite exposure.…”

“…Clinically related to asthma, Der p 23 sensitization had a high prevalence in HDM-sensitive people and was discovered in recent years ( 9 , 22 , 42 ). Walsemann and colleagues have shown that Der p 21 sIgE can affect the development of AD ( 43 ). In our study, the Der p 21 sIgE positive rate in the non-AD group was higher than in the AD group ( Figure 6D ), with the same trend for Der p 2 and Der f 2, indicating that Der p 21, Der p 2, and Der f 2 might play a more important role in respiratory allergic disease.…”
