In order to fill the know-do gap guidelines need to be appropriate and have adequate marketing strategies. There is a need for tailor-made guidelines for the specialised and the general care respectively. The domain conflicts have detrimental effects on the implementation of evidence based knowledge. Conclusions: There are several obstacles to the implementation of evidence based knowledge in clinical practice. Disincentives, financial restrictions and the local culture are the most powerful.
