Campylobacter at the Human–Food Interface: The African Perspective

The foodborne pathogen Campylobacter is a major cause of human gastroenteritis, accounting for an estimated annual 96 million cases worldwide. Assessment of the true burden of Campylobacter in the African context is handicapped by the under-reporting of diarrhoeal incidents and ineffective monitoring and surveillance programmes of foodborne illnesses, as well as the minimal attention given to Campylobacter as a causative agent of diarrhoea. The present review of the literature highlights the variability in the reported occurrence of Campylobacter in humans and animal food sources across different countries and regions in Africa. Campylobacter infection is particularly prevalent in the paediatric population and has been isolated from farm animals, particularly poultry, and foods of animal origin. The reported prevalence of Campylobacter in children under the age of five years ranges from 2% in Sudan to 21% in South Africa. In poultry, the prevalence ranges from 14.4% in Ghana to 96% in Algeria. This review also highlights the alarming trend of increased Campylobacter resistance to clinically important antimicrobials, such as ciprofloxacin and erythromycin, in humans and food animals in Africa. This review adds to our understanding of the global epidemiology of Campylobacter at the human–food animal interface, with an emphasis from the African perspective. Interinstitutional and intersectoral collaborations, as well as the adoption of the One Health approach, would be useful in bridging the gaps in the epidemiological knowledge of Campylobacter in Africa.

Campylobacter food poisoning is understated in developing countries. To determine the occurrence and antimicrobial resistance of this pathogen in Zagazig City, Sharkia Governorate, Egypt, a total of 286 samples from chicken (195), cattle (47) and human (44) were collected. Bacteriological examination of the collected samples revealed high prevalence rates of Campylobacter species from human stool, chicken and raw milk samples (90.91%, 86.15% and 82.98%, respectively). C. jejuni was recognized as the most frequently recovered species (63.29%). There were no significant effects on the prevalence of Campylobacter spp. based on the sample type (p = 0.54). Thirty eight campylobacter isolates were confirmed molecularly using PCR amplification of 23S rRNA gene. Furthermore, PCR targeting mapA and ceuE genes were then applied for the confirmation of C. jejuni and C. coli isolates, respectively. The antibiotic resistance results showed that all isolates (100%) were resistant to ampicillin, amoxicillin and erythromycin. On the other hand, the lowest resistance rates were detected against amikacin, impenem and cefoxitin (28.74%, 32.39% and 47.77, respectively). In total, 207 (83.81%) campylobacter isolates were MDR, while 38 isolates (15.38%) and 2 chicken C. jejuni isolates (0.81%) were XDR and PDR, respectively. One hundred and forty five antimicrobial resistance profiles were generated with an MAR index of 0.45 or greater. Our results suggested that the presence of MDR campylobacters in chicken and raw milk, specifically to erythromycin and/ or ciproﬂoxacin aggravates the human health alarm and emphasizes the necessity to educate the consumers about the safeness and the goodness of such foods.

Background: During food animal production, animals are exposed to, colonized by, and sometimes infected with bacteria that may contaminate animal products with susceptible and multidrug-resistant organisms (MDRO). The United States’ Organic Foods Production Act resulted in decreased antibiotic use in some animal production operations. Some studies have reported that decreased antibiotic use is associated with reduced MDRO on meat. Objectives: The aim of this study was to investigate associations of meat production and processing methods with MDRO and overall bacterial contamination of retail meats. Methods: Bacterial contamination data from 2012 to 2017 for chicken breast, ground beef, ground turkey, and pork chops were downloaded from the National Antimicrobial Resistance Monitoring System. Poisson regression models with robust variance were used to estimate associations with MDRO contamination and any contamination (adjusted for year and meat type) overall, and according to bacteria genus ( Salmonella , Campylobacter , Enterococcus , Escherichia coli ) and meat type. Results: A total of 39,349 retail meat samples were linked to 216 conventional, 123 split (conventional and organic), and three organic processing facilities. MDRO contamination was similar in conventionally produced meats processed at split vs. conventional facilities but was significantly lower in organically produced meats processed at split facilities ; 95% CI: 0.30, 0.63]. Meat processed by split vs. conventional processors had higher or similar MDRO contamination for all tested bacterial genera except Campylobacter ( ; 95% CI: 0.13, 0.64). The prevalence of any contamination was lower in samples processed at split vs. conventional facilities for aggregated samples ( ; 95% CI: 0.68, 0.73) and all meat types and bacterial genera. Discussion: Organically produced and processed retail meat samples had a significantly lower prevalence of MDRO than conventionally produced and processed samples had, whereas meat from split processors had a lower prevalence of any contamination than samples from conventional processors had.
