BACKGROUND In highly endemic countries, transmission and sub-clinical infection of leprosy are likely and the disease manifests itself in individuals without any known close contact with a leprosy patient. Health workers are social contacts belonging to the same network (the Health System) and some of them share the same social environment (nursing assistants) as patients with known patients and / or carriers. OBJECTIVE To identify ML Flow seropositivity among health professionals. METHODS We conducted a cross-sectional study using a serological survey with the ML Flow test in 450 health professionals (doctors, nurses and nursing assistants), in order to detect seropositivity in areas of high and low endemicity in municipalities from three Brazilian states (RJ, MS and RS). RESULTS The results showed general 16% seropositivity, higher in low endemic areas, regardless of whether there was direct care for leprosy patients. Paradoxically, a statistical association was observed between the area studied and seropositivity, as the place with the lowest endemicity (CA) had the highest seropositivity rate (p = 0.033). CONCLUSION The authors suggest these results are associated with a presence of an unspecified link to bovine serum albumin (BSA), carrier of PGL-1 in the ML Flow test, and recommend expanded seroepidemiological research utilizing tests with human and bovine albumin.

Objetivo: Avaliar a situação dos contatos intradomiciliares dos casos de hanseníase notificados no ano de 2012 em São Luis - MA. Métodos: Estudo quantitativo-descritivo-transversal, com coleta de dados utilizando-se instrumento estruturado. Resultados: A maioria dos contatos intradomiciliares eram mulheres (51,87%), idade entre 0 e 20 anos (40,29%), parentesco consanguíneo de 1º grau (54,92%), forma dimorfa (59,78%) e multibacilares (75,53%). Do total, 1880 (67,96%) não realizaram o exame dermatoneurológico. Entre os examinados, houve maior frequência no distrito Bequimão (28,48%), 59,82% eram mulheres, idade entre0 a20 anos (44,77%). Ao exame, 91,27% apresentou-se normal. Dentre os suspeitos, 36,73% tinham hanseníase, 58,21% possuíam cicatriz da 1ª dose da BCG e 59,98% foram encaminhados a vacinação. A prevalência da doença entre os contatos foi de 1,62%. As ações da vigilância de contatos classificaram-se como precárias em São Luis. Conclusão: A maioria dos contatos registrados não foi examinada, evidenciando necessidade de fortificação das ações da vigilância no município.

RESUMOA hanseníase faz parte das doenças prevalentes no Estado do Pará. As antigas Colônia do Prata, em Igarapé-Açu, e Colônia Marituba, na região metropolitana de Belém, ambas no Pará, onde eram hospedados pacientes portadores do mal de Hansen, retratam uma realidade histórica da época em que os pacientes eram segregados e separados das respectivas famílias em função do contágio a que estavam expostas. O advento do tratamento multidroga ou poliquimioterápico, no Brasil na década de 1980, mudou consideravelmente o quadro da endemia no País, com uma redução acentuada da prevalência da doença; embora o aparecimento de casos novos não apresente redução significativa nos países endêmicos, ainda assim, o quadro epidemiológico sofreu mudanças substanciais nos últimos 20 anos. O Instituto Evandro Chagas (IEC), órgão de pesquisa da Secretaria de Vigilância em Saúde, Ministério da Saúde, iniciou estudos epidemiológicos e realizou atividades de vigilância em saúde pública referentes à hanseníase na década de 1990, mais precisamente, em 1998, quando aplicou um método laboratorial simples, denominado ML Dipstick, o qual pesquisa anticorpos da classe IgM contra o glicolipídeo fenólico-I (PGL-I), um antígeno de parede celular específico do Mycobacterium leprae, em cinco municípios endêmicos para hanseníase no Estado do Pará. Este estudo de revisão tem como objetivo descrever a trajetória do desenvolvimento do Programa de Hanseníase no IEC, bem como avanços e perspectivas futuras, relevantes no contexto da endemia hansênica no Pará. Palavras ABSTRACTHanseniasis is a prevalent disease in Pará State, Brazil. The old settlements Colônia do Prata (Igarapé-Açu) and Colônia Marituba (metropolitan area of Belém), both in Pará, were home to patients with hanseniasis and portray a historical reality of patient segregation and separation from their families because of their contagious disease. Multidrug treatment (polychemotherapy) was established in Brazil during the 1980s and led to a significant countrywide decrease in the prevalence of hanseniasis. Although there were no significant decreases in the number of new cases reported in endemic countries, the epidemiologic framework dramatically changed during the past 20 years. Instituto Evandro Chagas (IEC) is a research agency of the Health Surveillance Secretariat of the Ministry of Health, which conducted epidemiologic studies and performed public health surveillance on hanseniasis during the 1990s. In 1998, a simple laboratory method called ML Dipstick was used for identifying IgM antibodies against the phenolic glycolipid-I (PGL-I), a Mycobacterium leprae-specific antigen; and those studies were conducted in five endemic municipalities located in Pará State. This report aimed to describe IEC's Hanseniasis Program, as well as provide future perspectives on hanseniasis in Pará State.
