Sonography and NMR Imaging in Rheumatoid Gonarthritis

Abstract: Sonography and NMR imaging of the knee joint make it possible to obtain a visual representation of the thickened synovial membrane occurring in patients with rheumatoid gonarthritis, even before inflammatory deformation is radiologically detectable. Both methods can expose effusions and Baker's cysts. In NMR imaging, an indication of the inflammatory genesis can be obtained from the extended relaxation time T1. Due to the good representation of soft tissues, prognostic evidence may be gained of whether a mainl… Show more

“…Compression with the transducer or the free hand enables mobilization of fluid and thus differentiation between effusion and synovial thickening. 1 If thickening of the synovial membrane is detected sonographically, measurement is possible ( Figure 1); sonographic measurements are highly correlated with those obtained by MRI. 2 Disease activity may be suspected if power Doppler sonography shows hypervascularization of the synovial membrane.…”

“…Ultrasound showed joint swelling in all cases. In case 1, synovial thickening could be seen through the presence of villosities of the posterior salience, as described by Wyld et al (5) and Hammer et al (6) . additionally, we probed the possibility of measuring synovial membrane thickness and found values ranging from 7 to 100 mm, consistent with those found by Bessmeltsev et al (7) : 6 to 8 mm and by Wyld et al (5) : 12 and 19 mm.…”

“…The US analysis reliably detects both abnormalities and it is thus considered an accurate imaging method for the study of synovitis [19,[27][28][29]. Previous studies demonstrated that magnetic resonance analysis of a single sagittal slice and US measurements of synovial thickness in the suprapatellar bursa are reliable and accurate methods for the quantification of the severity of the inflammatory process in RA and in Sj6gren's syndrome patients [19,30,31].…”
