Sonographic diagnosis of primary hydatid disease in the breast: The scroll sign

Abstract: Primary hydatid disease of breast is a rare entity and is caused by the larval stage of Echinococcus granulosus. The disease is more commonly seen in sheep-rearing populations and imaging plays a pivotal role in its diagnosis. Several imaging signs have been described in relation to hydatid cysts. However, the "scroll sign," due to the infolding of the endocyst, has rarely been encountered, with only one similar case in literature, in which the imaging findings were inconclusive. We present a case of primary h… Show more

“…Only few papers reported follow-up studies -there were no recurrences after a mean follow-up of 38 months (3-60). 5,[10][11][12]16,[22][23][24][25][26][27][28] Additionally, two cases with alveolar echinococcosis were reported in the literature (from Bulgaria and Turkey). 29,30…”

“…6 A fluid level with moving echoes may be seen due to hydatid sand composed of hooklets, membranes, and debris, which gives the appearance of the "Snowstorm". 8 Another sign, known as the "congealed water lily" sign, has been described as strongly suggestive of hydatid cysts. It appears as an increased echogenicity of cyst fluid (secondary to its increased viscosity) and an immobile germinal membrane, giving it a solid appearance.…”

“…The complete detachment of the endocyst from the pericyst leads to a floating membrane which produces the “water lily” sign 6 . A fluid level with moving echoes may be seen due to hydatid sand composed of hooklets, membranes, and debris, which gives the appearance of the “Snowstorm” 8 . Another sign, known as the “congealed water lily” sign, has been described as strongly suggestive of hydatid cysts.…”
