Race records were obtained for 31 thoroughbred racehorses that were treated conservatively (rest, improvement of fitness and/or use of a tongue tie) for dorsal displacement of the soft palate, and for 62 matched control horses. Their racing performance, based on prize money won, was compared for three races run before and after the disorder was diagnosed. Nineteen (61 per cent) of the conservatively treated horses had higher earnings after conservative treatment than before treatment, compared with 27 (43.5 per cent) of the controls. There was a significant (P=0.049) increase in the earnings of the horses treated conservatively, but there was no significant difference in the earnings of the control group.

ZusammenfassungDurch transendoskopische Operationsverfahren (minimal invasive Chirurgie: MIC) lassen sich die Risiken von chirurgischen Maßnahmen im Bereich der oberen Atemwege des Pferdes reduzieren. Allerdings sind nicht alle Erkrankungen mit dieser Technik befriedigend therapierbar. Zum Einsatz kommt eine kostenaufwendige Technik, die ein Endoskop mit Videosystem, diverse transendoskopische Instrumente, ein Hochfrequenzchrrurgiegerät und einen Chirurgielaser beinhaltet. In den oberen Atemwege können damit Erkrankungen der Nasengänge, der Nasenmuscheln und der Nasennebenhöhlen, der Luftsäcke, des Pharynx und des Larynx behandelt werden. Die transendoskopische Chirugie ist besonders geeignet bei der Behandlung von kleineren Nasengangs-und Siebbeintumoren, bei der Therapie von Luftsackerkrankungen (Abszess, Tympanie, Mykose) und bei der Entfernung von Subepiglottiszysten. Bei der Therapie der Dorsalverlagerung des Gaumensegels und des Epiglottis-Entrapment sind transendoskopische OP-Verfahren eine gute Alternative zu konventionell chirurgischen Verfahren. Die endoskopische, partielle Resektion der Stimmfalte bei einer Kehlkopflähmung stellt eine komplikationsarme Ergänzung zu bestehenden Operationsmethoden (Ventrikelektomie und Laryngoplastik) dar, durch die in vielen Fällen eine ausreichende Leistungssteigerung erzielt werden kann.Schlüsselwörter: Minimal invasive Chirurgie, Endoskopie, Laser, obere Atemwege, Pferd Possibilities and limitations of minimal invasiv surgery in upper airway diseases of the horse Through the use of transendoscopic operative techniques (minimal invasiv surgery) it is possible to reduce the risks associated with surgical procedures of the upper respiratory tract. lt is, however, not possible to satisfactorily treat all cases with this technique. The technique is expenstve, and involves the use of an endoscope with a video system, various transendoscopic instruments, a high{requency generator and a surgical laser. Its appltcation in the upper respiratory tract includes the treatment of diseases of the nasal passages, the ethmoid turbinates and the sinuses, as well as the guttural pouches, the pharynx and the larynx. Transendoscopic surgery is particularly well suited for the treatment of small nasal passages tumors and ethmoid hematomas, in the therapy of guttural pouch abscess, tympany and mycosis and in the removal of subepiglottic cysts. In the treatment of the dorsal displacement of the soft palate and in epiglottic entrapment the transendoscopic operative technique represents a sound alternative to conventional surgical techniques. The endoscopic, partial resection of the vocal fold in the case of Hemiplegia laryngis represents an uncomplicated addition to the already establishedtechniques (ventriculectomy and laryngoplasty), through which in many cases, a sufficient increase in performance can be achieved. keywords:Einleitung

BackgroundPalatal dysfunction (PD), which encompasses palatal instability (PI) and intermittent dorsal displacement of the soft palate, is the most common performance‐limiting upper respiratory tract obstruction in young Thoroughbred racehorses. Soft palate thermocautery (SPT) performed under general anaesthesia is a routinely performed procedure for PD in some countries, but the procedure for and outcome of SPT performed under standing sedation has not been published.Objectives(1) To describe a technique for SPT performed under standing sedation; and (2) to assess post‐operative performance in horses compared to controls using the Racing Post rating (RPR), British Horseracing Authority official rating (OR), Performance Index and Earnings.Study designRetrospective case series.MethodsMedical records were reviewed for all horses that had SPT performed under standing sedation following topical and local infusion of lidocaine hydrochloride into the rostral soft palate, and that were identified to have PI by overground endoscopy. Two matched controls were identified for each case. The median RPR, OR, Performance Index and Earnings for the three pre‐operative and three post‐operative races were compared.ResultsNo significant differences were identified between the SPT (n = 23) and Control groups (n = 46) for baseline characteristics or outcomes.Main limitationsRetrospective study design, small sample size.ConclusionsNo significant differences in racing performance were identified between horses that had SPT performed under standing sedation and controls.
