Soluble Triggering Receptor Expressed on Myeloid Cells and the Diagnosis of Pneumonia

Abstract: In patients receiving mechanical ventilation, rapid detection of sTREM-1 in bronchoalveolar-lavage fluid may be useful in establishing or excluding the diagnosis of bacterial or fungal pneumonia.

“…19 Patients, 18 years or older, hospitalized in an ICU were prospectively enrolled if they met the following criteria: 1) need for mechanical ventilation and 2) clinical suspicion of bacterial pneumonia. Bacterial pneumonia was suspected if a newly developed and persistent infiltrate on chest radiography was associated with at least one of the following: purulent tracheal secretions, body temperature of at least 38.3˚C, and leukocytosis (>10 g/l) or leukopenia (<4 g/l).…”

“…sTREM-1 levels were measured with an immunoblot technique, as described previously (9,10). One hundred microliters of each plasma sample was dotted onto nitrocellulose membrane, dried, and then coated in PBS supplemented with 3% BSA and incubated with monoclonal anti-human TREM-1 (21C7) (3) for 60 min.…”

“…Furthermore, TREM‐1 is constitutively expressed in a‐granules and mobilized at the membrane on platelet activation 15. sTREM‐1, a soluble form of TREM‐1 shed from the membrane of activated phagocytes, used to be identified as a marker to distinguish infectious from noninfectious inflammatory states, and its significance in infectious diseases is clearly confirmed 16, 17, 18, 19…”
