Solitary plasmacytoma of bone with oncogenic osteomalacia: recurrence of tumour confirmed by PET/CT. A case report with a review of the radiological literature

Abstract: We describe a case of oncogenic osteomalacia secondary to solitary plasmacytoma of the vertebral body of T3. The patient presented with symptoms of hypophosphataemia. Following the initial diagnosis, the lesion was surgically resected with good results, although several follow-up procedures, including bone grafting, were necessary to stabilize the thoracic spine. The lesion recurred almost 15 years after its initial resection, again presenting with hypophosphataemia and neurological symptoms suggestive of loca… Show more

“…All cases, including ours, involved surgical intervention as a part of therapy. Chemo- and/or radiotherapy were used in less than half of spinal TIO cases [4, 68, 11]. In the present case, resection of tumor with spondylectomy using anterior retroperitoneal approach and L-4 corpectomy was curative.…”

“…5,6 Current evidence points to FGF-23 as the causative humoral factor responsible for this type of osteomalacia. 9 A second imaging modality shown to be sensitive at times when octreotide scintigraphy has not been successful is 18 F fluorodeoxyglucose positron emission tomography (PET) scan with CT. 10,11 More recently, a single case report has combined the specificity of octreotide scintigraphy and sensitivity of PET/CT into Ga DOTANOC PET/CT. In the setting of hypophosphatemia, the 24-hour Patients presenting with new-onset osteomalacia, particularly those with atypical presentations or few risk factors, should incur a workup for potential reversible causes.…”

“…Radiographically solitary lytic lesions with associated bone pain have been described in cases of plasmacytoma [18]. In these cases, there is a predilection for male patients and involvement of the axial skeleton with absence of hypercalcemia, anemia, or renal impairment [8]. Langerhans' cell histiocytosis could not be ignored as patients with bone involvement present with pain and swelling of the affected area.…”
