Solitary lymphangioma of the spleen: dynamic MR findings with pathological correlation

Abstract: Solitary lymphangioma of the spleen is an extremely rare, benign vasoformative tumour that typically shows no notable enhancement on imaging studies. Few descriptions of the MRI features have been reported and, to our knowledge, the findings on dynamic gadolinium-enhanced MR images have never been described. We report a case in which MR images showed diffuse and prolonged enhancement of a splenic mass. On histological correlation, splenic lymphangiomas with abundant fibrous stroma may account for this specific… Show more

“…2015;139:278-282; doi: 10.5858/ arpa.2013-0656-RS) L ymphangiomas are benign malformations of the lymphatic system, usually found in the neck (75%) and axilla (20%) 1 and less commonly encountered in the orbit, mediastinum, adrenal gland, kidney, bone, omentum, gastrointestinal tract, retroperitoneum, liver, and pancreas. 2,3 They were first described by Rodender in 1828; however, the first case involving the spleen was reported in 1885 by Frink. 4,5 Between 1939 and 2010, only 189 cases of splenic lymphangiomas were reported in the literature.…”

“…Areas of high signal intensity may be shown on T1-weighted images if internal hemorrhage is present or if the lesions contain a large amount of proteinaceous fluid [1]. The intervening hypointense septa always reflect the presence of fibrous connective tissue [11]. At microscopic examination, lymphangiomas are composed of multiple vascular channels that are lined with a single layer of endothelium and filled with proteinaceous fluid.…”

“…Imaging characteristics are hypo-or anechoic lesions detected by ultrasound, low attenuation and non-enhancing masses detected by CT, and high T2 and low T1 signal intensity on MRI [5] . Different imaging characteristics of splenic lymphangiomas, such as a solid-cystic mass, mildly increased echogenicity on ultrasound, and fill-in contrast enhancement on delayed CT images and MRI, are also described in some cases [6][7][8] . Nevertheless, a solitary splenic lymphangioma is difficult to differentiate from a splenic cyst/angioma or a metastatic tumor.…”
