Solid carcinoma of the thyroid gland.Analysis of 53 cases

Abstract: Of a series of thyroid carcinomas, approximately 10% have been of the solid type. The histologic pattern of solid thyroid carcinoma is distinctive: I t is the only type that contains amyloid and it is unmixed with other types. The incidence in males and females is essentially equal. Solid carcinoma tends to metastasize to the cervical lymph nodes and the mediastinum. T h e diseases with which it is associated brings this type into special perspective among thyroid cancers. Its histologic pattern is not a guide… Show more

“…The frequency of the finding of chronic diarrhoea during medullary carcinoma of the thyroid was 29.3% in William's series (1966) and 28-3% in the series of Ibanez et al (1967) which included 41 and 51 cases respectively.…”

“…Since the first descriptions by Williams (1966) and Bemiel, Bouvry, Cattan, and Prost (1966) diarrhoea associated with medullary carcinoma of the thyroid has been reported on several occasions (Bemier, Savoie, Garnier, Cattan, Bouvry, Rambaud, Bognel, and Prost, 1967;Hillemand, Gilbert-Dreyfus, Debray, and Cattan, 1967;Ibanez, Cole, Russel, and Clark, 1967;Davis, 1967;Cope and Williams, 1967). In these publications the authors attempted mainly to determine the frequency and pathogenesis of this association.…”

“…Consequently, the macroscopical aspect and the clear-cut borders between the two parts of tumour 1 at first glance favour collision of two separate tumours. Some contact areas, however, revealed a mixed pattern, which has not been observed in the few cases of coexisting medullary and papillary carcinoma reported by other authors (Ibanez et al 1967;Justrabo et al 1978;Lamberg et al 1981;O'-Neil and Lomas 1984;Ishida et al 1985;Gero et al 1989). The morphological pattern closely resembled the mixed thyroid tumour published by Eusebi et al (1990), with a smaller papillary and a larger oat-celMike, calcitonin-free component, separated by clear-cut borders.…”

“…It is possible that this improvement w-as related to the fall in circulating calcitonin since it lhas been suiggested that the diarrhea occurring in association wvith medullarv carcinoma of the thyvroid may be due to calcitonin inhlibition of sodium cand water absorption bv the intestine (21). On the otlher hand, hligh contenits of serotonin and( prostaglandins lhav e been fotund in some thyroid medullarv carcinomas and both of these compounds are knoxvni to stimulate smooth muscle (22)(23)(24). The possibility that, in addition to its effect on calcitonin, magniesiumil might influence the secretion or sy-nthesis of other bioactive substances produced by tllhroid medullary carcinomas, including prostaglandi ns, serotonin, ACTH, and histaminase, is currently tunder investigationi.…”
