Software for Dosage Individualization of Voriconazole for Immunocompromised Patients

Abstract: eThe efficacy of voriconazole is potentially compromised by considerable pharmacokinetic variability. There are increasing insights into voriconazole concentrations that are safe and effective for treatment of invasive fungal infections. Therapeutic drug monitoring is increasingly advocated. Software to aid in the individualization of dosing would be an extremely useful clinical tool. We developed software to enable the individualization of voriconazole dosing to attain predefined serum concentration targets. … Show more

“…TDM should be considered a component of routine care, although further work is required to identify drug exposure targets. The population PK model can now be used to construct algorithms for individualized dosing, as we described previously (33,34).…”

“…The voriconazole cartridge included the structural model equations relating input (voriconazole dosing) to output (voriconazole plasma concentrations) and the discrete joint probability distribution of the values of the equation variables (pharmacokinetic parameters) in the population, consisting of support points. The software used the cartridge and each patient's weight and voriconazole dosing and concentration data to find the least biased and most precise dosage regimen relative to a target concentration, as previously described (29).…”

“…In this paper, we present a validation scheme that is similar to what we devised for the adult controller software (29) but that is now applied to children. The algorithm is able to track measured concentrations in the past and either to calculate doses to achieve future target drug exposures or to predict future exposures for a given dose.…”

“…Optimised dosing using an estimation of an individual's PK to identify the optimal dosage has not been undertaken with anti-bacterial agent. Pilot studies, within other infectious diseases areas, have demonstrated with HIV drugs in paediatric patients and in the management of invasive pulmonary aspergillosis with voriconazole (Neely and Jelliffe 2010;Hope et al 2013). The impact of dosage adaptation on patient outcome requires further study.…”
