Softening Our Approach to Discussing Prognosis

Abstract: We tend to cringe when we hear "So, how much time do I have, doc?" Yet prognostic discussions are a core skill of being a compassionate physician, preparing patients and families to live with serious illnesses, and enabling informed medical and personal decisions. The first challenge of prognostic communication is the inherent and unavoidable uncertainty, that the exact trajectory of health (or illness) is unknowable. Thus, clinicians are inaccurate prognosticators, overestimating by up to a factor of 5. 1 We … Show more

“…Denying doubt promotes premature closure in the decision process and increases potential for error . Clinicians feel uneasy answering questions about prognosis because of poor accuracy of predictions . On the other hand, the explosion of online information, often marketing “breakthroughs” in medicine, increases patients' expectations for more accuracy, leading to requests for more testing and second opinions.…”

“…While physicians are often unaware of its power, empathic listening is key to alleviate the patient's emotional distress from uncertainty. Rather than seeking unrealizable precision, a recommended tool for the clinician is to combine hopes and worries—saying, for instance, “I am hoping that you have a long time to live with your heart disease and I am also worried that the time may be short, as short as a few years.” When clinicians empathically allow for hope, patients are less overwhelmed and more easily tolerate prognosis . To express hope in the face of serious illness, a key skill is paradoxical thinking—the ability to simultaneously imagine opposite scenarios .…”

“…Palliative care specialists are trained to communicate around these diffi cult points of transition, but generalist clinicians already involved in the patient's care can also sensitively explore patients' prognostic awareness as it relates to the hospital transfer. 9 In the Table, The phrases mentioned suggest language that is helpful in broaching such discussions, which assesses the patient's illness understanding, hopes, and worries. Asking about patients' hopes for their illness enables clinicians to quickly know some of their most important priorities.…”
