Comparison between textured silicone implants and those bonded with expanded polytetrafluoroethylene in ratsComparison between textured silicone implants and those bonded with expanded polytetrafluoroethylene in rats ABSTRACT Purpose:Comparison of the inflammatory reaction promoted by textured silicone implants and that caused by the implant bonded with e-ptfe. Methods: One-hundred and fifty rats were divided into three equal groups (control, silicone, and bonded e-ptfe). These groups were subdivided into five groups, according to the second operation, i.e., 7,30,60,90 and 180 days. Histology of the peri-implant tissue was analyzed by morphometry with blood count (neutrophilos, lymphocytes, macrophages, fibroblasts and capillaries). Results: Comparison of subgroups 7,30,60,90, 180 days: -neutrophils: silicone: > in subgroup 7 days; bonded e-ptfe: > in subgroups 7 and 30 days; -lymphocytes: silicone: > in subgroup 7 and 180 days; bonded e-ptfe: > in subgroup 180 days; -macrophages: silicone: > in subgroup 7 and 60 days; bonded e-ptfe: > in subgroup 7,30 and 60 days; -fibroblasts: silicone: > in subgroup 30 and 60 days;-vascular volume: silicone: in subgroup 7, 60 and 90 days; bonded e-ptfe: > in subgroup 7 days. Conclusions: The acute stage of the inflammatory response was more severe and irregular in the silicone implant; both the silicone implant and the silicone bonded with e-ptfe promoted chronic inflammatory reaction and weak foreign body inflammatory response. RESUMO Objetivo:Comparar a reação inflamatória provocada pelo implante de silicone texturizado, com aquela causada por este recoberto com PTFE-E. Métodos: Foram utilizadas 150 ratas, divididos em três grupos igruais (controle, silicone e recoberto PTFE-E). Os grupos foram subdivididos em cinco subgrupos, ou seja, 7, 30, 60, 90 e 180 dias, de acordo com a data do segundo ato operatório. Disruption of the thrombospondin 2 gene (Thbs2) in mice results in a complex phenotype characterized chief ly by abnormalities in fibroblasts, connective tissues, and blood vessels. Consideration of this phenotype suggested to us that the foreign body reaction (FBR) might be altered in thrombospondin 2 (TSP2)-null mice. To investigate the participation of TSP2 in the FBR, polydimethylsiloxane (PDMS) and oxidized PDMS (ox-PDMS) disks were implanted in TSP2-null and control mice. Growth of TSP2-null and control skin fibroblasts in vitro also was evaluated on both types of disks. Normal fibroblasts grew as a monolayer on both surfaces, but attachment of the cells to ox-PDMS was weak and sensitive to movement. TSP2-null fibroblasts grew as aggregates on both surfaces, and their attachment was further compromised on ox-PDMS. After a 4-week implantation period, both types of PDMS elicited a similar FBR with a collagenous capsule in both TSP2-null and control mice. However, strikingly, the collagenous capsule that formed in TSP2-null mice was highly vascularized and thicker than that formed in normal mice. In addition, abnormally shaped collagen fibers were observed in capsules from mutant mice. These observations indicate that the presence or absence of an extracellular matrix component, TSP2, can inf luence the nature of the FBR, in particular its vascularity. The expression of TSP2 therefore could represent a molecular target for local inhibitory measures when vascularization of the tissue surrounding an implanted device is desired.

Цель. морфологическими методами оценить реакцию тканей организма и процессы деградации силиконовых имплантатов после их применения в клинических условиях.Материал и методы. методами световой микроскопии изучали соединительнотканные капсулы, сформированные вокруг различных силиконовых маммоимплантатов.Результаты. выявлено, что после имплантации объемного мягкого инородного тела (силиконовые маммоимплантаты), этот материал длительное время присутствует в тканях, отграниченный фиброзной капсулой. постепенно капсула вследствие деятельности миофибробластов для минимизации объема чуже-родного тела сжимается, внутренняя ее поверхность деформируется и приобретает волнообразный вид с множеством выростов или выпячиваний внутрь. соединительнотканная капсула вокруг силиконовых им-плантатов состоит из плотной (наружной, основной) и рыхлой (внутренней, пограничной) тканей. плотная часть капсулы изолирует инородное тело от тканей организма. в рыхлой соединительнотканной части про-исходит постепенное разрушение (фрагментация) и поглощение материала имплантатов фагоцитами. со временем силикон мигрирует в ткани капсулы и за ее пределы, где поглощается фагоцитами. в результате миграции силикона в ткани и поглощения его фагоцитами объем маммоимплантата постепенно уменьша-ется и капсула сокращается для соответствия объему инородного тела, при этом внутренняя часть капсулы волнообразно деформируется. значительная выраженность гранулематозного воспалительного процесса и образование толстой капсулы с признаками фиброзирования являются неблагоприятными прогности-ческими признаками, указывающими на более высокую вероятность развития в дальнейшем различных осложнений.Заключение. Чем инертнее материал имплантата для живого организма, тем меньше он будет стиму-лировать макрофагальную реакцию. задача создания новых имплантатов сводится к поиску максимально биоинертных материалов, достаточно прочных к сжатию капсулой и к фрагментированию и, при этом, сходных по эластичности с нормальной тканью молочной железы.Ключевые слова: силикон, имплантация, контрактация капсулы, гигантские клетки инородных тел, гранулематозное воспаление Objectives. Using morphological methods to estimate the reaction of the organism tissues and degradation processes of silicone implants after their application in clinical conditions.Methods. The connective tissue capsules formed around various silicone mammary implants were studied by applying the method of light microscopy.Results. It was revealed that after implantation of a volume soft foreign body (silicone mammary implants) this material presented in the tissues for a long period of time delimited by a fibrous capsule. Gradually the capsule compresses owing to the activity of myofibroblasts for minimization of the volume of an alien body, its internal surface is deformed and obtains a wavy form with a set of outgrowths or protrudes inside.
