Socioeconomic Aspects of Spinal Imaging: Impact of Radiological Diagnosis on Lumbar Spine-Related Disability

Abstract: Low back pain presents a difficult problem for patients and their doctors. The symptom affects the majority of people at some point in their lives and usually has a benign natural history. However, as a society, we consume tremendous resources to diagnose and treat painful spinal disorders. Magnetic resonance imaging (MRI) facilitates the "medicalization" of low back pain due to its exquisitely sensitive depiction of pathoanatomy. Unfortunately, many of these findings are present in normal subjects. Radiologis… Show more

“…a scan or MRI, results should always be interpreted in light of the clinical findings. Unnecessary use of these highly sensitive examinations will produce numerous false positive results[ 13 ], which can create a labelling effect for the clinician and his patient that can in itself contribute to a less favourable prognosis[ 15 ].…”

“…41 MRI may, in fact, facilitate the "medicalization" of LBP, due to its visually exquisite depiction of pathoanatomy. 8 In fact, it is questionable whether the term pathoanatomy or abnormality appropriately describes what could be considered nonpathological or normal, age-related or degenerative changes. For example, among asymptomatic persons 60 years or older, 36% had a herniated disc, 21% had spinal stenosis, and over 90% had a degenerated or bulging disc.…”

“…Það er í samraemi við erlendar niðurstöður sem sýna almennt lítið samraemi milli einkenna og niðurstaðna segulómunar, en þó einna helst samraemi milli verkjar niður í fót og rótarþrýstingsgreiningar frá hryggþófa við segulómun. 9,10,24 Þetta ósamraemi milli einkenna og segulómunar undirstrikar að varasamt er að byggja sjúkdómsgreiningu bakverkja eingöngu á segulómun, heldur þarf samþaett mat sem byggir einnig og ekki síst á nákvaemri, klínískri skoðun. Einnig sýnir þetta hve torvelt það er að greina vandamál í lendahrygg, sem aftur endurspeglast í miklum fjölda óskilgreindra bakverkja.…”

“…Þar sem brjóskútbungun getur þróast í brjósklos, er í sjálfu sér ekki óeðlilegt að þegar slíkar breytingar eru byrjaðar geti fleiri fylgt í kjölfarið. 9,12 Einkenni brjóskútbungana og brjóskloss eru keimlík og mikill meirihluti segulómniðurstaðna staðfesti meinsemd (brjóskútbungun og/eða brjósklos) í hryggþófa þar sem spurt var um brjósklos, en einnig hjá rúmum helmingi þeirra sem ekki höfðu slíka spurningu á beiðni. Hér var því nokkurt ósamraemi á milli rannsóknarniðurstaðna segulómunar og spurninga á beiðni varðandi hryggþófavandamál í heild.…”
