Background Social innovation in health provides innovative solutions to address healthcare delivery gaps and it relies on engaging community. However, little is known about factors that influence community engagement in health research. The study sought an in-depth understanding of elements of community engagement in social innovations identified by the global social innovation in health initiative network. Methods The study employed a sequential mixed methods study approach, which involved a series of semi-structured interviews with 27 social innovators followed by an online survey among social innovation researchers. Semi-structured interviews were recorded, transcribed and analyzed using NVivo 11. Themes identified from the qualitative study informed the development of a survey instrument on community engagement. The survey data were analyzed using STATA version-14 and descriptive statistics were presented. Results Community engagement in social innovation was mostly utilized during during delivery of interventions (213; 79.2%), intervention identification and design (179; 66.5%), and problem identification (167; 62.1%). About half of the social innovations (135; 52.1%) had collaborate or shared leadership level of community engagement. We noted differential participation of community stakeholders at different stages of community engagement in social innovations. Social innovations built capacity of community stakeholders to offer health services to communities. Community engagement in social innovations resulted into intervention acceptance, and improved sustainability and community ownership of interventions. Conclusion The study shows moderate community engagement in social innovations in health. Creating strong governance structures as well as co-creating interventions with communities are vital towards achieving high community engagement in social innovation in health.

Innovaciones sociales para mejorar la saludLas innovaciones sociales en salud son procesos en los que las comunidades y los innovadores, en forma colaborativa, crean e implementan soluciones prácticas a los problemas de la salud y sus factores determinantes. Usualmente, influyen sobre los sistemas y servicios de salud locales (1,2).Existen múltiples definiciones de innovación social, las cuales concuerdan en que se producen cambios resultantes de la creación colectiva de conocimientos desarrollados con la comunidad, o por ella, cambios que tienen un enfoque holístico, deben ser sostenibles y dejar capacidades instaladas; el liderazgo suele ser compartido entre la comunidad y otros actores (2).En la práctica, las innovaciones representan la traducción integrada del conocimiento en la que, de principio a fin, se aprende, se ajusta y se mejora, siguiendo un diseño centrado en las personas. Con frecuencia, estas innovaciones se insertan en los planes, programas y políticas públicas (2,3). Estos conceptos los ilustraremos con ejemplos.Para conocer las innovaciones, se requiere mirar procesos y resultados hilando las historias que las definen; son ricas en contexto y complejidad en su evolución (2,4). La Social Innovation in Health Initiative (SIHI) y el Café Virtual del Centro Internacional de Entrenamiento e Investigaciones Médicas (CIDEIM), coleccionan historias así 1,2 . En Colombia, las familias afectadas por el Zika -apoyadas por investigadoras del Instituto Nacional de Salud-aportaron evidencia para mejorar la calidad de los servicios de salud. Las evidencias sustentaron nuevas políticas públicas acordes con las necesidades de esas familias y niños afectados por el virus del Zika (5).Mamás del Río es una innovación peruana que ilustra bien los elementos de la innovación social y su apuntalamiento en la investigación, con una historia y un contexto documentado que llega a diferentes audiencias (6-9).En el 2015, investigadores de la Universidad Peruana Cayetano Heredia visitaron el río Marañón en la Amazonía peruana, una de las regiones con los peores indicadores de salud materna y neonatal (10). Junto con la Dirección Regional de Salud (DIRESA) de Loreto, visitaron 13 comunidades en el distrito de Parinari. Se ganaron la confianza de las comunidades y aprendieron de la gran prevalencia de partos domiciliarios atendidos sin personal entrenado, y del pobre acceso a la atención de salud de calidad Volumen 42,No. 3

Research on social innovations in health has increased in recent years. However, little training is geared toward enhancing social innovation research capacity. Most health training for low- and middle-income countries (LMICs) is developed by individuals in high-income countries, disregarding LMIC researchers’ wisdom and insights and the communities’ needs. Our team organized a multi-phase investigation involving a series of surveys and co-creation group discussions to assess individuals’ training needs that directly informed a subsequent co-created training workshop series. We conducted a Hennessy–Hicks Training Needs Assessment among the Social Innovation in Health Initiative (SIHI) network and formed a co-creation group comprising SIHI fellows to design related training workshops. We ran a final evaluation survey and analyzed the workshop series’ strengths, weaknesses and threats. Descriptive and thematic analysis were employed to analyze survey data and open-ended responses. The final evaluation survey captured data from 165 learners in 35 countries, including 26 LMICs. Most participants (67.3%, 111/165) rated the training workshop series as excellent, and 30.3% (50/165) rated it as good on a five-point scale. The need for writing research grants and manuscripts was rated the highest priority. Learners were interested in community-engaged research and diversity, equity and inclusion. This workshop illustrated how co-creation could be an effective tool for developing training materials tailored for LMIC researchers. We also offer a template for conducting a needs assessment and subsequent training workshops for LMICs. The ground-up, locally developed courses may be more effective than externally developed training programs intended for LMICs.
