SMASH standardised perioperative management of patients operated with acute abdominal surgery in a high-risk setting

Abstract: Objective of the study: Emergency laparotomy and other high-risk acute abdominal surgery procedures have a high mortality rate. The perioperative management of these patients is complex and poses several challenges. The objective of the study is to implement and evaluate the outcome of protocol-based standardised care for patients in need of acute abdominal surgery in a Swedish setting. NÄL is a large county hospital in Sweden serving a population of approximately 270,000 inhabitants. The study seeks to determ… Show more

“…It is therefore of great importance to further study this group of patients. Our results present the first part of the SMASH project [ 21 ], an ongoing study at the Northern Älvsborg County Hospital (NÄL), NU-Hospital Group (Trollhättan, Sweden) exploring whether or not the introduction of a standardised perioperative protocol for high-risk abdominal surgery can affect outcome in a Swedish context.…”

“…Encouraged by these changes, in 2017 the NU Hospital Group (Vastra Gotaland County in southwestern Sweden) developed a protocol for standardized management. The SMASH (Standardized perioperative Management of patients operated with acute Abdominal Surgery in a High-risk and emergency setting) study started in February 2018 17 . The aim of the study was to investigate whether standardized perioperative management improved postoperative outcomes after emergency laparotomy in a Swedish context.…”

“…Looking forward, there is obviously a drive to reduce morbidity and mortality and lead to a better outcome by raising awareness and implementing change. The SMASH trial from Sweden is ongoing, seeks to determine whether a new standardised perioperative protocols consisting of rapid start of operation in emergency abdominal surgical procedures leads to a better outcome compared with the present standard in Swedish routine health-care [25]. In China, the Abdominal Pain Center model has been in place for several years and has demonstrated a better management and in-hospital clinical outcomes of patients with abdomen.…”
