Smart suction device for less blood trauma: a comparison with Cell Saver

Abstract: In this experimental set-up, the Smart suction system avoids damage to red cells and affects platelet count less than the Cell Saver system which induces important blood cell destruction, as any suction device mixing air and blood, as well as severe hypoproteinemia with its metabolic, clotting and hemodynamic consequences.

“…This was probably caused by both reasons, namely, platelet activation in extracorporeal circulation and higher fragility of platelet membrane. 17 In the CCS group, platelet salvage was minimal (5.1%), confirming the finding of other studies that 95% to 99% of the platelets are removed by cell saver. The significant difference in cell and protein salvage in the two groups is because the semipermeable membrane of the dialyser does not allow large molecules such as blood cells and proteins to cross the barrier.…”

“…Loss of proteins and coagulation factors is not significant till blood loss is less than 1 L but may lead to significant hypoproteinemia and coagulopathy once blood loss is greater then 3 L. Preserved serum albumin levels help maintain blood pressure and prevent pulmonary complications by maintaining the plasma oncotic pressure. In an animal experiment by Mueller et al, 17 they found that as serum proteins level fell to less than 15% of preoperative level, it was impossible to maintain blood pressure despite infusion of large volume of crystalloids. Similarly, normal level of serum immunoglobulins translate into reduced risk of postoperative infection.…”

“…To achieve a nearly perfect equilibrium, the Smart system was developed [9]. By this means, cardiotomy blood is being suctioned into a reservoir that is directly connected to the tubing system but separated by a manual clamp, which is being released for fluid balancing, if needed [11].…”

“…Removing cardiotomy suction further reduced neutrophil activation [188]. Cardiotomy suction is a signifi cant source of red cell hemolysis and a potential source of cytokine and leukocyte activation [189] secondary to air-blood intermixing. A new technology-Smart System-reduces air-blood intermixing by turning on only in the presence of blood [190].…”
