Purpose: To compare visual outcomes and aberration outcomes in small incision lenticule extraction (SMILE) versus femtosecond laser LASIK (FS-LASIK). Methods: This prospective, comparative, nonrandomized clinical study included 68 eyes of 37 patients receiving SMILE and 55 eyes of 30 patients receiving FS-LASIK between December 2011 and January 2013 at the Fudan University Eye and ENT Hospital (Shanghai, People's Republic of China). Patients were followed up at 3, 6 months and 5 years after surgery. Main outcome measurements included uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, central corneal thickness (CCT), total corneal refractive power (TCRP) and wavefront aberrations. Results: About 98% (49/50) of treated eyes in the SMILE group and 95% (39/41) in the FS-LASIK group had a postoperative logMAR UDVA of 0 or better. Spherical equivalent (SE) after 5 years was À0.01 AE 0.35 D in the SMILE group and À0.23 AE 0.41 D in the FS-LASIK group. A regression of À0.02 AE 0.39 D in the SMILE group and À0.12 AE 0.32 D in the FS-LASIK group was observed between 6 months and 5 years postoperative time-points. TCRP increased by 0.39 AE 0.38 D in the SMILE group and 0.45 AE 0.49 D in the FS-LASIK group between 6-month and 5-year time-points. However, no statistically significant difference was found between the SMILE and FS-LASIK groups in terms of ΔSE, ΔCCT and ΔTCRP between 6 months and 5 years postoperative time-points. Conclusion: Myopic regression was observed in terms of TCRP but not in subjective refraction. No statistically significant difference in stability was found between SMILE and FS-LASIK.

Femtosecond laser (FSL) is a near-infrared laser that can create reliable and reproducible tissue cutting with minimal damage to adjacent tissue. As the laser can also create incisions with various orientations, depths, and shapes, it is expected to be a useful tool for anterior segment surgery, such as cornea, refractive, and cataract surgery. In this review, the authors will introduce the application of FSL in various anterior segment surgeries and discuss the results of studies regarding the efficacy and safety of FSL in cornea, refractive, and cataract surgery. Experimental studies regarding the potential use of FSL will also be introduced. The studies discussed in this review suggest that FSL may be a useful tool for improving the prognosis and safety of surgeries of the anterior segment.

Background and RationaleThe idea of reshaping corneal morphology basing on intrastromal lenticule implant starts from the introduction of Femtosecond Laser (FSL) in corneal surgery, in particular in refractive surgery. FSL is an ultrafast pulse laser that allows the dissection of tissues inducing a process of photodisruption and plasma cavitation formation [1].Differently from the excimer laser photoablation, which vaporizes tissues, FSL performs stromal cuts by collagen lamellae separation; in this way the corneal stroma can be remodelled according to the desired shape. FSL was at first mainly used in LASIK surgery for flap creation; subsequently in refractive surgery the procedure of lenticule extraction (ReLEx; Carl Zeiss Meditec, Jena, Germany), was introduced in alternative to LASIK surgery to correct refractive errors [2]. This innovative approach in refractive surgery is based on stromal refractive lenticule removal performed with one single FSL laser. The refractive lenticule is created with pre-determined power within the anterior corneal stroma.ReLEx surgery is now performed without a flap creation and the refractive lenticule is extracted through a small stromal incision. This technique, Small Incision Lenticule Extraction, rapidly spread in clinical practice and is known as SMILE [3].To treat myopia the refractive lenticule presents a concavo-convex profile. The anterior surface is determined by the planar cap dissection while the posterior surface curvature correlated with the amount of refractive error to be corrected. The higher thickness of the lenticule is located the center and gradually reduces in the periphery [2,3].
