Background: Obesity is strongly associated with atherogenic dyslipidemia phenotype and is an independent risk factor of cardiovascular disease (CVD). Besides body mass index (BMI), there are various obesity indices, namely waist circumference (WC) to detect central obesity, and percentage of body fat (%BF) using bioimpedance analysis (BIA) to detect peripheral-central obesity. The aim of this study is to determine which obesity index is better in predicting dyslipidemia. Methods:This cross-sectional study involved 99 professionally active doctors working at tertiary hospital, from January to March 2021. Obesity was measured by obesity indices such as BMI, WC, and %BF using BIA. After that, the serum lipid profile was then measured. Dyslipidemia is a disorder of lipoprotein metabolism, including lipoprotein overproduction or deficiency that may be manifested by high total cholesterol, low-density lipoprotein (LDL) cholesterol, triglycerides, and low high-density lipoprotein cholesterol (HDL) cholesterol. Results:Of the total 99 subjects, 49.5% were male, while 50.5% were female. The prevalence of obesity based on BMI, WC, and %BF using BIA was 57.6%, 74.8%, and 72.7%, respectively. Obesity based on BMI has 9.8 times the risk of having low HDL cholesterol levels (odds ratio (OR) = 9.814, 95% confidence interval (CI): 1.213 -79.379) and 4.6 times of having high triglycerides levels (OR = 4.618,). Meanwhile, central obesity based on WC has 3.1 times the risk of having high LDL cholesterol levels (OR = 3.100, 95% CI: 1.170 -8.218). On the contrary, the results of the analysis on obesity based on %BF on lipid profile were not significant.Conclusions: Obesity based on BMI and WC measurements are better than %BF in predicting dyslipidemia.

APO-B merupakan apolipoprotein terbesar dari fraksi Very Low Density Lipoprotein (VLDL). Hipertrigliseridemia membuat hati memproduksi lebih banyak VLDL dan LDL kaya trigliserida yang akan dihidrolisis untuk menghasilkan LDL padat yang lebih kecil (sd-LDL) melalui Cholesterol ester transfer protein (CETP) dan lipase hati. Sekitar 90% dari APO-B adalah LDL, membuat rasio K-LDL/APO-B sebagai indikator partikel LDL. Penelitian ini dilakukan di Laboratorium Klinik Prodia Wonogiri. Penelitian ini merupakan penelitian observasional analitik yang dilakukan dengan menggunakan desain cross sectional. Pengambilan sampel kuota dilakukan untuk memilih 31 sampel berdasarkan kriteria yang telah ditentukan, yang diperoleh data koefisien korelasi antara trigliserida dengan APO-B sebesar 0,135 dan trigliserida dengan rasio K-LDL/APO-B sebesar 0,819. Nilai sig trigliserida dengan APO-B adalah 0,469 dan trigliserida dengan rasio K-LDL/APO-B adalah 0,000. Trigliserida tidak ditemukan memiliki hubungan yang signifikan dengan APO-B, namun trigliserida sangat berkorelasi dengan rasio K-LDL/APO-B. Sedangkan arah hubungan trigliserida dengan APO-B dan dengan rasio K-LDL/APO-B adalah negatif.

Background: Small dense low-density lipoprotein (sdLDL-C) is an atherogenic lipoprotein. Increased sdLDL-C concentration was hypothesized to be associated with obesity and diabetes mellitus (DM).Objectives: The study aimed to determine the association between sdLDL-C, central obesity, and DM among Indonesian adults, controlled by personal and clinical parameters.Materials and Methods: This study used secondary data from Basic Health Research 2013 of the Ministry of Health, which applied a cross-sectional study design. For this purpose, 30,548 subjects aged 19-79 were analyzed. The sdLDL-C was performed by using Sampson Formula derived from conventional lipid panels. As investigated by Sampson, the formula referred to cLDL-C (calculated LDL-C) and ElbLDL-C (estimated large buoyant LDL-C).Results: There was a positive association between sdLDL-C and central obesity (OR: 3.94; 95% CI: 3.13-3.89), as well as sdLDL-C and DM status (OR: 1.98; 95% CI; 1.43-2.75) after adjusting the personal and clinical parameters.Conclusion: This study demonstrated that the increment of sdLDL-C level and central obesity affected DM status in Indonesian adults. It implies that the sdLDL-C was a potential biomarker to assess the risk of DM.Keywords: Central obesity, Diabetes mellitus, Hypertension, Indonesian adult, sdLDL-C
