Small Cell Neuroendocrine Carcinoma of the Oropharynx Harbouring Oncogenic HPV-Infection

Abstract: Small cell carcinoma/neuroendocrine carcinoma (SCNEC) of the oropharynx is uncommon. Recently, an association has been reported between oropharyngeal SCNEC and high-risk human papillomavirus (HPV) infection. While HPV infection confers a better prognosis for oropharyngeal squamous cell carcinoma, HPV infection does not appear to influence the biological behaviour of SCNECs, which are generally associated with poor clinical outcomes. We document two cases of SCNEC arising in the oropharynx with evidence of high… Show more

“…Thus, an HPV-positive form of small cell carcinoma would not have been readily appreciated at the writing of the 2005 edition of the Blue Book. Like small cell carcinomas of the lung, these HPV-positive small cell carcinomas of the oropharynx are associated with cigarette smoking, high grade cellular features, expression of neuroendocrine markers, and an aggressive clinical behavior including widespread dissemination and poor survival [24][25][26]. This aggressive clinical behavior is in sharp contrast to non-small cell carcinomas of the oropharynx where the presence of HPV is associated with highly favorable survival rates.…”

“…Subtypes 16, 18 and 33 have been identified at this site 13 14. HPV has also been associated with small cell neuroendocrine carcinoma of the cervix with subtypes 18 and 16 reported in up to 40% of cases 15.…”

“…HPV‐related small cell carcinoma of the oropharynx is a newly recognized variant with few cases reported in the literature . This study presents one of the first comprehensive cytologic characterizations of this entity and includes 6 patients.…”

“…In contrast to SqCC variants, HPV‐related small cell carcinomas follow an aggressive clinical course and are almost uniformly fatal. To date, fewer than 2 dozen cases of HPV‐related small cell carcinoma of the oropharynx have been reported in the literature . Despite the growing use of fine‐needle aspiration (FNA) cytology for the evaluation of neck masses and masses at other sites and the adequate performance of these specimens for HPV testing, there are no reports in the literature describing the cytomorphologic features of this entity .…”
