Small bowel obstruction and perforation attributed to tubo-ovarian abscess following 'D’ and 'C’

Abstract: We report the case of a young woman who was admitted because of small bowel obstruction and localized peritonitis following a dilatation and curettage ('D’ and 'C’) of uterus in abortion. As infection, like tubo-ovarian abscess may complicate any abortion, it seems wise to ensure that it does not exist prior to performing a 'D’ and 'C’.

“…Surgical procedures of the female genital tract, abdominal, or vaginal, place the patient at increased risk for PID, with ~15% of pelvic infections occurring after procedures that break the cervical mucus barrier. 18 , 19 Such infections have been reported after dilatation and curettage, 18 , 19 oocytes vaginal retrieval for IVF, 19 , 20 Essure insertions even few years later with Essure acting either as a nidus or as a conduit for infection, 21 and pipelle sampling. 19 …”

“…Intussusception in adults is rare and always require a laparotomy as it is commonly due to an underlying malignancy such as lymphoma or carcinoma. Any inflammatory mass such as diverticular, appendiceal, tubo-ovarian ileo-caecalCrohn'setc, may cause small bowel obstruction by blocking the ileocaecal valve or by causing a localized ileus [18,19]. The narrowest part of the small bowel is the terminal ileum which is the preferred site of obstruction by most of the luminal causes.…”

“…In patients whose comorbidities may predispose them to worse outcomes if complications arise (i.e., poor glycemic control, immunodeficiency, poor nutritional status), a more aggressive initial approach may be appropriate. In addition, maintaining a high level of concern about the involvement of adjacent structures including the large and small bowels should be upheld throughout their care, even after initial management [ 8 ]. LBOs are surgical emergencies and require close observation of the cecum as its intrinsic characteristics make it particularly susceptible to perforation.…”
