Sleeve gastrectomy telementoring: a SAGES multi-institutional quality improvement initiative

Abstract: Background Sleeve gastrectomy is a relatively new procedure that developed as a result of rapid innovation in the field of bariatric surgery. As with any newly developed operation, there is a learning curve that potentially can be associated with higher morbidity. Real-time surgical mentoring reduces the learning curve effect but can be time intensive for the mentor. The aim of this initiative was to evaluate the feasibility, effectiveness, and satisfaction of surgical telementoring for laparoscopic sleeve gas… Show more

“…Other solutions have been proposed, for example, the SAGES telementoring, which allows surgeons to reach the plateau of maximum performance more quickly by "correcting" intraoperative gestures, thanks to experts who can follow the procedure remotely. An evaluation is proposed via this device; unfortunately, it is only subjective since it is left to the expert's free appreciation [22] and always on a patient.…”

“…The study concluded surgical telementoring may enable physicians to safely perform two-incision leg fasciotomy in remote environments [74]. Another study from California reported telementoring for guidance of surgical trainees through sleeve gastrectomy procedures was a highly rated learning tool by both mentors and mentees [75]. Recently a case report of two patients successfully undergoing laparoscopic high-anterior and lowanterior resections, respectively, using telementoring has demonstrated the potential of the 5G network [76].…”

“…As a result, other surgical fields have used telementoring to provide the expertise that in-person mentoring may provide while allowing rural or remote hospitals access to high-quality mentorship [12,15]. For example, in laparoscopic sleeve gastrectomy, telementoring was shown to be feasible, and it resulted in positive evaluations from participating surgeons, as well as integration of new techniques into their surgical practice [8]. In Switzerland, a formal telementoring program safely introduced endovascular repair of abdominal aortic aneurysms in a remote center with good integration of the surgical technique into future surgical practice [11].…”
