Sleep nasendoscopy: a technique of assessment in snoring and obstructive sleep apnoea

Abstract: It appears that uvulopalatopharyngoplasty (UVPP) is a reliable procedure for reducing snoring, but much less reliable when used as a treatment for OSAS. This is thought to be because of poor patient selection in that the site of the problem is not always the site of the operation. We present the technique of sleep nasendoscopy which allows direct visualization of the site or sites of obstruction in a sleeping patient. Our study has shown that there are patients with obstructive sleep apnoea syndrome (OSAS) in … Show more

“…Surgical treatment methods have been developed to decrease the vibration in this region. [1] Although uvulopalatopharyngoplasty (UPPP) has been the most frequently used surgical method, tissue stiffening surgeries have become more popular because they cause less morbidity and provide more patient comfort. Radiofrequency ablation Because radiofrequency is a frequently preferred older method, many studies have been carried out upon it and its side effects and long-term results are well-established.…”

“…Vibration, however, most frequently occurs at the level of the soft palate. [1] For that reason, the key strategy for preventing snoring is to prevent vibration of the soft palate. Surgeries at the soft palate level can include those that decrease tissue volume, harden the tissue through fibrosis, or both.…”

“…Good measurement characteristics include accuracy, low testretest variability and low interrater variability. Sleep endoscopy in the naturally asleep patient was introduced in 1978 by Borowiecki et al [20] and in the asleep or sedated patient by Croft and Pringle [21]. The evaluation requires pharmacologic induction of sedation and flexible fiberoptic endoscopy to visualize the upper airway obstruction and/or snoring.…”

“…Croft and Pringle described the technique in 1991 [8]. Sleep endoscopy is a technique that consists of pharmacologically inducing sleep in Sleep Disordered Breathing patients to identify the area(s) of vibration and collapse in the upper airway.…”

“…This inaccurate assessment leads to inappropriate surgical intervention [6,7]. As obstructive sleep apnea can occur at many levels (multilevel obstruction), and the main regions of obstruction are the soft palate and hypopharynx (actually corresponding to the hypopharynx and the retro-lingual portion of the oropharynx) [8].…”
