Sleep disturbance in men receiving androgen deprivation therapy for prostate cancer: The role of hot flashes and nocturia

Abstract: The results of the current study suggest that the association between ADT and sleep may be partly explained by nocturia and hot flash interference. Future studies should examine behavioral and pharmacologic interventions to address these symptoms among ADT recipients. Cancer 2018;124:499-506. © 2017 American Cancer Society.

“…This beneficial effect occurred despite the study not selecting men for troublesome hot flushing or excluding men continuing to receive other medications for hot flushing. Yet, despite previous reports associating increased hot flushes with sleep disturbance (33) and reduced quality of life in men undergoing ADT (34,35), these benefits did not translate into a detectable improvement in overall quality-of-life scores. This disparity may be due to a lack of power because of the short duration of this study or because baseline quality of life was already relatively high in our cohort by comparison with other men with PCa (36).…”

“…2b, difficulty sleeping and hot flashes are on the edges of their respective symptom clusters. While we cannot demonstrate causality, it is known that the occurrence of hot flashes disrupts patients’ sleep 66,67 . If our findings are confirmed in an independent sample, future NAs can evaluate for causality and test interventions to reduce symptoms across clusters.…”

“…Cognitive impairment could also be associated with other known psychiatric adverse effects of ADT in men with PCa, such as depression [ 22 , 23 , 24 ] and reduced sleep quality [ 25 , 26 ]. Depression has been documented to increase in men with PCa, with a prevalence between 10% and 40% [ 27 , 28 ] that might be related to multiple factors, such as age, comorbidities, socioeconomic status, erectile dysfunction or the disorder itself [ 28 , 29 , 30 ].…”

“…Some studies have concluded that poor sleep quality appears in approximately one-third of the men treated with ADT, but the underlying physiological mechanism is unclear [ 35 , 36 ]. Among other factors, it has been related to hot flashes, nicturia and emotional distress and to the pharmacological treatment of these adverse effects [ 26 , 37 , 38 , 39 ].…”
